Question: hi
I had a vaginal ultra sound
It showed that I have three fibroids
Also stated that I had heterogeneous debris within the endometrial canal with no definite polyp


I had the ultrasound in my fifth day of the menstruation
I have regular periods with some heavy bleeding in the second and third day of the period

Thanks for writing to us. Followings are my comments:

1) I have reviewed your attached ultrasound scan and it reveals MULTIPLE (3 in numbers) fibroids - ONE of them is QUITE LARGE (7.1 x 6.9 x 7.2 cm). You need proper treatment.

2) In this case, treatment can be initiated with haemostatic agent combined with oral progesterone pill for 3- 6 months duration. Levonorgestrel intrauterine devices (LNG-IUD) are HIGHLY effective to control bleeding and persisting symptoms.

Some other medical treatments are available to SHRINK fibroid tumor and before going to surgical option. They include DNAZOL, GnRH analogue, Selective Progesterone Receptor Modulator (Ulipristal acetate, Asoprisnil), Progesterone antagonist (Mifepristone) etc.

3) Minimally invasive treatment like Radio Frequency Ablation and Non-Invasive treatment like Uterine artery embolization are also available to SHRINK fibroid and improve patient's condition.

4) In case of failed medical therapy or if you prefer conservative treatment, then MYOMECTOMY (removal of fibroid only) can be done.

If you have completed family, then BEST surgical option is Subtotal or Total Hysterectomy WITH or WITHOUT Oophorectomy (removal of Ovary).

5) I suggest you to consult with your GYNECOLOGIST for clinical examination and necessary investigation for proper assessment to determine RIGHT choice of treatment in this case. You should start taking IRON supplement with your daily diet to combat future risk of anemia.

1) Uterine fibroid is a BENIGN tumor. The malignant version of a fibroid is extremely uncommon and RARE.

2) If you want to increase fertility, then medical treatment with Progesterone supplement can be initiated for first 3-6 months.

However, myomectomy (surgical) would be the BEST treatment if the tumor is operable. Kindly, consult with your gynecologist for complete clinical assessment to determine the right protocol.

at the end of the report they wrote this Heterogeneous debris within the endometrial canal, possibly blood products. No focal
polyp identified. Consider clinical correlation with endometrial biopsy as clinically
indicated.
Is it dangerous or is it due to having the ultrasound during my period
I do not have anemia shall I take iron supplement or depend on food only
Is having Progesterone supplement is enough with no need for operation
Is there is any herbs that help in shrinking fibroids

Thanks for additional query.

1) Heterogeneous debris is MOSTLY related with endometrial shedding or blood products as you were on the period. It is NOT dangerous thing.

2) If you want to preserve fertility or pregnancy, then treatment option is LIMITED. Progesterone supplement is usually given to shrink fibroid and control bleeding. It is an INITIAL treatment to be started before going to other option.

Surgical or other minimally invasive treatment may be NECESSARY, depending on the response of medical treatment and patient profile.

3) IRON added foods will be helpful if you DON'T have anemia at present. Kindly, consult with your doctor for proper assessment.
