What Does "high CPK-278 And Squamous Epithelial Cells" Indicate?

Question: I'm a 76yr.old woman, having autoimmune issues, 1 rheum said I have Sjogrens & 2 rheum say no Sjogrens. Blood test just came back with high CPK-278 and Squamous Epithelial Cells 6-10 a few. I an having some spotting though scant, and I've had a total hysterectomy 30+ years ago. I have severe muscle cramps and diagnosed with fibromyalgia. Could you please give me an answer as to what I might be expecting with these blood tests. Also, recently have diagnosis of neuropathy.

I am Dr. Prakash H M and I will try to help you with some comments.

1. Blood tests show a CPK level of 278 and appear to be total CPK. CPK appears in fractions like CPK-MB (cardiac), CPK-MM (muscle) and CPK-BB (Brain). M stands for muscle and B stands for brain. If specific levels are available for each fraction, possible cause can be identified.
However, we can rule out a few things.
You do not have any signs or symptoms of stroke (Brain infarction) or heart attack (myocardial infarction), So causes secondary to brain or cardiac muscle can be ruled out.
Total CPK now could be possibly due to skeletal muscle involvement. The normal values varies from 38-174 U/L. A level of 278 usually does not signify any significant pathology. It could be secondary to Intramuscular injection, alcoholism or even exercise. If you were my patient, I would not be worried about this. If you still have suspicions, you can ask your doctor to get CPK fractions laboratory tests done. Fibromyalgia does not raise CPK levels.

2. Occasional epithelial cells are seen in normal urine sample. If epithelial casts were to be noted, it could have been significant.

3. Sjogrens syndrome presents with dry eyes and dry mouth. Since you have not reported any such symptoms, I cannot comment about its status. If you can provide further history or laboratory tests report (you can upload it using the site's uploader), I can make my comments on it.

4. Spotting, even though scant, needs to be viewed seriously. In your age, atrophic vaginitis could be a plausible explanation. I usually advise topical estrogens for it. You can meet your doctor for confirmation and prescription. However, do get a gynecological opinion to rule out any other diseases.

5. In view of you recent diagnosis of neuropathy, I would suggest you to get appropriate care of foot and maintain a nutritious diet.
