What Does High Alkaline Phosphatase Indicate?

Question: I have an 8-1/2 yr old granddaughter I'm raising. She's been diagnosed with bi-polar, mild autism, possibly some signs of schizophrenia - so obviously our hands are pretty full with her behaviors. On a routine physical recently, they thought they detected some goiter or enlarged thyroid but the testing came back normal on that. I'm worried about her blood work though, with being NPO her glucose was nearly 100, her creatinine was 0.5, alkaline phos are at 202, and total bili is at .19. She takes Prozac and 10 mg Adderoll, but these numbers (I am an RN - not a Dr) are outside where they should be. The only number off on her hematology is the MCV value being low at 80.1. The Dr. who ordered the test doesn't seem concerned now that it's not thyroid related - but I am. Diabetes and kidney disease run on my husband's side of the family, and I don't like the numbers on the chemistry side of the test. Am I worrying too much?

I agree with you in Alkaline phosphatase being high and need for further investigation. Such rise in this enzyme level can b seen in conditions occluding the liver portals. Either there could edema, damage or a stones occluding the bile radicals in the liver. You may have to get a CT scan of the abdomen to rule out the same.

Creatinine 0.5 and blood sugar 97 even on fasting is not a matter of concern. These vary a lot and would be within range if tested again.

The same applies to the MCV value.
