Question: hello,dr...my one day baby is in nicu...and dr is saying that his potassium is high in kidney...plz tell me is there any major problem....his weight is 3.4 kg...

Hi, I am a neonatologist working in NICU and can understand the worry you are going through.

There are few causes of increased serum potassium on day 1 of life. First comes kidney cause. If the baby has sufferred asphyxia during delivery blood supply of kidney often goes down causing injury to kidney. But with supportive management 99% recovers with time. But there are other congenital diseases of kidney which can cause prolonged problem and long term treatment is required.

other causes are

1. If the baby suffered from any bruise during delivery it can also increase potassium level.
2. Any Endocrinological problem related to kidney and adrenals can be the cause. They are congenital disease.
3. Excessive jaundice or hemolysis in the body which is normal to some extent can also give rise to this problem.

Lastly what i want to impress upon is that if the pregnancy was uneventful and no abnormality was noted in antenatal ultrasonography, you dont require to worry much. The cause would be a transient one and will go away.

Antenatal ultrasonography are the ultrasound scan done during pregnancy to monitor the growth of the baby and detect any abnormality. I think all the reports were normal.

Uneventful pregnancy means there was no problem during pregnancy like injury or trauma to mother, high fever or any serious medication disease requiring hospitalisation

dr..all sonography done during pregnancy was normal....my wife goes through a minor nailtone surgery at 8th month....and she has thyroid problem....

hello dr...my baby dr said his pottassium is normal now...but my baby sleeps a lot....and dosent feed directly from mother...he ignores it or usually sleeps at that time....is there any problem...

Initiation of feeding takes time in some babies. Dont worry the baby will start feeding directly from mother in no time. I dont think there is any problem at present as this is a common finding... Dont worry be happy.

Baby sleeps over 16 -18 hrs a day. Baby who had a stress like high potassium are more likely to sleep more. Please dont worry,give some time the baby will recover completely.

Sorry to hear that aspiration of milk has occured, in aspiration some amount of milk enters into the lungs which causes problem. The severity of the problem depends on the amount of milk aspirated by the baby. Generally very small amount of milk is aspirated and the problem that it creates goes away in 3-5 days. Only routine care and sometimes Oxygen is required as treatment. Antibiotics are also often prescribed if the baby is in NICU... He will be fine , dont worry.
