What Does Increased Liver Parenchymal Echotexture Suggest?

Question: Hello Doctor,

My recent LFT shows the following abnormalities:
ALP 133, GGT: 104SGOT 36, SGPT 39. The levels of ALP and GGT has been high for more than a year Last test in Sep 2013 shows ALTP AT 125 and GGT at 85. Other LFT functions were normal. I am diabetic but under control by taking Metformin 500-850 mg for the past 12 years. I am also having bp controlled by ramipril 5 for past 12 years. How do I reduce ALP and GGT levels. Ultra sonography reveals mild diffuse increase in echogencity.No focal parenchymal lesions and no evidence of intra/extrahepatic bilary tree dilation noted. Portal vein appears normal. Other ultra sound whole abdomen functions are normal.

Kindly advise.

Increased liver parenchymal echotexture suggests fatty liver.

Fatty liver is a reversible condition and it can be reversed by diet and life style changes.
Increased GGT suggests liver injury.It mainly indicates alcoholic liver injury.
It may take 3-4 months to reverse the findings.

In such conditions,I suggest tablet ursodeoxycholic acid 300 mg twice daily for three months to my patients. It helps in regeneration of liver cells. Discuss about this your doctor.

Increased ALP is non specific and it increases in many conditions.

You should avoid red meat and high fat milk products.Avoid alcohol if you take it.
You should avoid fried and junk food.
Take lot of fruits and vegetables.You should also increase your physical activity in the form of walking and exercise.

Take anihypertensives and antidiabetic medicines regularly.

Other findings are within normal limit.

Get well soon.

Dear Doctor,

Thank you for your prompt reply. I am not an alcoholic as I take only a glass of red wine once in a week. Kindly suggest whether I should discontinue that during the medication period. I am also a vegetarian and normally walk half an hour atleast 4 to 5 days in a week. I seldom eat outside may be once in 3 months. I have oatmeal and fruits for breakfast, rice and vegetables with curd for lunch and roti, dal and vegetables for dinner( some rice or suji preparations like Idli, Dosa or Upma occasionally). With this regimen still I got adverse results in my LFT. So I would like to know could there be any other reasons that could have triggered the abnormal values. I am asking so that I can preempt the recurrence of this in future.

Also is there any natural extracts that can be taken to control GGT and ALP.

Kindly advise.

You should try to quit red wine completely even if you take it occasionally.

You also need viral markers examination to exclude any viral cause.However,normal SGPT doesn't support viral cause.

In such condition,I suggest to take ursodeoxycholic acid tablet.

You don't worry.You will be alright,but you should quit red wine completely.
