What Does "inverted Cd4:cd8 Ratio, And 23% LGL's In Lymphocytes" Mean?

Question: flow cytometry states cd45 dim makes up 4% of the population that are mostly negatibe immunoreactivity. Also negatibe for cd34 and cd117. Degenerating t cells also noted, inverted cd4:cd8 ratio, and 23% LGL's in lymphocytes. Causes ?

Hi

It means out of the total lymphocytes found in blood 23 % are large granular lymphocytes (LGL) which are type of white blood cell that contains granules with enzymes that can kill tumor cells or microbial cells.

Normal range of LGL is 10% to 15% of lymphocytes.

Increased LGL can occur in rheumatoid arthritis and in viral infections.

T cell degeneration can occur in viral infections such as Herpes simplex virus, Mycobacterium and Listeria.

Fungal infections can also cause T cell degeneration.

I will advise you to get your blood tested for viral and fungal markers by your doctor.

What about the blast cells ? I should have mentioned that my LGL's have been elevated since January. I was also diagnosed with Mycosis Fungoides at MD XXXXXXX I also have high Uric acid. Do I need to be concerned about any types of lymphoblastic leukemias ? T cell disorders ?

Hi

Blast cells are immature cells found in bone marrow.

They are not fully developed, and therefore, do not yet carry out any particular function within the body.

In normal humans up to five percent of the cells found in bone marrow are blast cells.

When a higher percentage of them are found further testing may be needed as this is an indication for acute myelogenous leukemia and myelodysplastic syndrome

In your case blast cells are only 3.4% which is within normal range and you should not be concerned for lymphoblastic leukemias and T cell disorders.

The high uric acid is due to Mycosis Fungoides.

Blast cells are not typically found in the circulating blood of healthy individuals.

The presence of blast cells found in blood is therefore very suspicious of leukemia.

You need to do bone marrow test to rule out leukemia.

A sample of bone marrow is needed from your hipbone.

The bone marrow is removed using a long, thin needle.

The sample is sent to a laboratory to look for leukemia cells.

Consult with your doctor for the above test.
