Posted on Tue, 28 Apr 2015
Question: Had irregular bleeding--post menopause (63 years old) went to dr and got the following results-two fibroids that are mildly hyper vascular means--endometrium is thickened as well 11.6mm--have a D & C scheduled and a hysterosocpy as well. I am wondering if they should remove the two 'fibroids' as well--

Brief Answer:
No need to remove fibroids now

Detailed Answer:
Hi W8vrider, I have gone through your question and understand your concerns. Post menopausal bleeding can be due to many causes like senile endometritis, post menopausal hormonal replacement therapy, but the most important and common cause is malignancy of the lining of the uterus (endometrial carcinoma) and cancer of the cervix.
Thick endometrium more than 4 mm and fibroids in post menopausal patient with complains of bleeding is more suggestive of increased estrogen activity in the body which very greatly increase the chances of endometrial carcinoma. To rule it out, you have been posted for D&C and hysteroscopy so that a biopsy can be send and a definitive diagnosis can be made.
Fibroids are not the cause of bleeding in you, the thick endometrium is the cause. Therefore in initial management, there is no need to remove fibroids. When the diagnosis would be confirmed, you may be posted for hysterectomy.
