I have gone through your query and I have also reviewed the Image.
I can see an area of redness in the axillary fold.
This is suggestive of a fungal infection/ tinea also considering the fact that it is itchy.
Body folds trap moisture and are warm thus providing an ideal environment for fungus to proliferate.
If I was the treating doctor I would suggested you to use a topical antifungal cream. I would advice you to use it twice daily.
Clotrimazole 1% cream is an OTC topical antifungal. You may use it twice daily.
Clotrimazole 1% is also available along with topical steroid e.g beclomethasone dipropionate 0.025% as a combination cream.
This combination would be more suitable over plain topical antifungals itching is bothersome because steroids are antiinflammatory and would take care of itching and redness.
An Oral antifungal e.g weekly fluconazole can also be added.
Both topical and oral antifungals need to be continued for 4 weeks
An OTC oral antihistamine e.g cetrizine 10 mg once daily will provide you symptomatic relief from itching.
Once the condition resolves using an antifungal dusting powder in body folds regularly would prevent re-infection.
