What Does Low Antral Count Mean?
Question: Hi doctor, my AMH is 0.3 what is my chance of natural conception ? Does low reserve mean I do not ovulate after every cycle?

Low antral count means that the number of oocytes has decreased. Your AMH values are in the very low range and in such a case, natural conception is mostly not feasible and also considering your age, I would suggest IVF.
You are probably not ovulating every cycle, too. You can have additional methods of testing for antral reserve along with a trans-vaginal sonogram. I would ask you to see an infertility specialist for proper evaluation and further management.

What could be a possible reason for the decreased AMH levels? I always had ovulatory pain on the 10th day, which I am not experiencing for last 8-9 months. How can I know if I am ovulating in a month? Will these medicines help? Thanks

The several causes for decreased AMH levels are-
-advancing age
-low ovarian reserve, which may be caused by high body mass index or BMI, torsion or surgical removal of some part or whole of the ovary, endometriosis causing cysts, tumors, adhesions in the pelvis, immunological conditions and chromosomal or genetic anomalies.
If you do not have any history of infections or cysts or surgery, then age should be considered. The age of fertility often runs in families; often when a mother attains menopause at an early age, the daughter also does.
To know if you are ovulating, there are several ways-
-you can start a basal body temperature chart, the simplest way to show the rise in temperature observed at ovulation, it can also be the earliest indicator of conception evidenced by the continuation of the rise if fertilization occurs.
-follicular studies
-ovulation test through kits.
The medications given are to induce ovulation and release the ovum. To know if you respond well, some more tests like Estradiol levels, antral follicle count, FSH and Inhibin B levels also should be done before embarking on any real treatment as the existing little amount of eggs cannot be unnecessarily wasted.
A clomiphene citrate challenge test can be done.
