Brief Answer:
If you have no urine control then surgery is necessary.

Detailed Answer:
Hi,
Thanks for your query.

I'm Dr. Sudhir Kumar,

After going through your query I concluded that you are having central small disc protrusion at level of L5, S1 level. It means the intervertibral disc between lumbar 5 and sacral1 vertibra's small part is coming out from its place. Mild bilateral neural foraminal stenosis means there is mild narrowing of neural foraminal (hole through which nerve coming out) on both sides. No definite nerve root impingement means nerves are not compressed. Mild spinal narrowing means neural foraminal volume when compared to the normal levels was 66% or more.It generally occurs when only 1-2 mm of disc is protruded. Looking only to MRI report there is possibility of nonoperative treatment to be suceessful.

But because you has urinary and bowel symptoms so probably surgery is essential. Symptoms of cauda equina syndrome (lower back, rectal or genital pain, micturition disturbances, loss of bowel control, perianal sensory disturbances, impotency) make surgical intervention is essential.

You can discuss with your treating Doctor about it.

Hope I have answered your query.
