What Does Missed Periods Indicate?

default
Posted on Wed, 14 Oct 2015
Question: Hi doc,
My wife has recently conceived a baby girl. She's five months old. However, we had an unprotected intercourse last week.
Her last menstrual period is on 22nd August, 2015
We had intercourse on 15th September. I've pulled out too. I haven't ejaculated inside. However, she has missed her period date and we're tensed as we are not ready for another baby anytime soon.
She's lactating now. Kindly suggest us what is the safest method to remove it.

Brief Answer:
Please take a pregnancy test if she misses her period.

Detailed Answer:
Hello
Thanks for writing to us with your health concern.
First of all please take a pregnancy test.
If it is positive, then please let me know.
I will guide you further regarding the procedure for termination of pregnancy.
But first please take a pregnancy test after one week, and then let me know.
As such she is lactating, and you had sex just before the due period, so chances of pregnancy are very less, plus there was no ejaculation.
So if she misses her period, please take a test and let me know.
Otherwise do not worry, use regular contraception in future.
All the best.
PLease feel free to discuss further.

Hi doc,
Thanks for the clarification.
We have checked today and it turned negative.

However, thanks to the movies, she is very tensed about the process of termination of pregnancy and how it would affect her milk secretion.

Doc, since we have to wait for another 5 days to conduct another test, can you please tell us the intricacies in termination process. Will it affect her lactation. Will it affect her health.

I hereby kindly request you to clear the air in her mind and give her some confidence. She's thinking a lot doc.

Thank you,
XXXX

Brief Answer:
Have explained the process for your benefit.

Detailed Answer:
Hello again.
Thank you for the update.
Actually there is no point thinking about what you do not even know will happen.
Chances of pregnancy are less, so why worry and think negative ?
If you still wish, I will guide you about the termination process.
First and foremost, medical abortion should NOT be attempted without medical supervision.
1. Even if the home pregnancy test is positive, the pregnancy needs to be confirmed by a gynecologist and exact dating needs to be done, since these pills work only up to specific gestational age, and not beyond that. So do not act just after taking a pregnancy test at home.
2. You need to rule out possibility of ectopic pregnancy (pregnancy outside the uterus) which will also be done by a specialist, as taking medical pills in that scenario is disastrous and life threatening.
3. Medical abortion has also got chances of severe life threatening bleeding at times, for which you need to be under medical care.
4. It has a 5 % failure rate, following which you require surgical termination, which might be needed in an emergency also.
5. Unsupervised medical abortion might cause severe infection, retained products which maybe undetected. Hence it is important to have a follow up visit with your gynecologist two weeks AFTER taking the pills, at which it would be confirmed that the pregnancy was successfully and completely terminated.

That said and done, if this procedure is supervised by a doctor, it is quite safe and fairly successful.
I would give you a few guidelines about medical termination: Each pack contains 5 tablets -
1 tablet of Mifepristone…200 mg
4 tablets of Misoprostol, each containing Misoprostol…200 mcg

Mifipristone tablet is taken orally and 48 hours later, 2 tablets of Misoprostol are taken orally or vaginally.
Bleeding might begin at any time after taking Mifepristone or Misoprostol, but generally starts within 6 - 8 hours of taking Misoprostol tablets.
Another 2 tablets of Misoprostol are taken 24 hours later, irrespective of whether the bleeding has started or not.
If you vomit within 4 hours of using any pills, the dose has to be repeated..

For women at 49–63 days of gestation, if abortion has not occurred 4 hours after administration of misoprostol, a second dose of misoprostol 400 mcg may be administered vaginally or orally (depending upon preference and amount of bleeding).

The patient should return for a follow-up visit approximately 14 days after the administration of mifepristone. This visit is very important to confirm by clinical examination or ultrasonographic scan that a complete termination of pregnancy has occurred.

Patients who have an ongoing pregnancy at this visit have a risk of foetal malformation resulting from the treatment. Surgical termination is recommended to manage medical abortion treatment failures
Also, the dose might vary depending on body weight or other risk factors present in the patient, so as stressed by me earlier, please go for this procedure ONLY AFTER consultation, and examination by a specialist; else you place the person undergoing this at great risk.

Now, regarding her worry about lactation and her health.
It is best not to breastfeed for 6 hours after taking each tablet in the kit.
She can resume breastfeeding 6 hours after taking the tablets.
There is no long term negative effect on her health, as long as she conducts the process under medical supervision, as I explained.

I hope this will reassure her.
