Posted on Mon, 21 Jul 2014
Question: 34-yr-old female who recently had head CT scan to rule 34-yr-old female who recently had head CT scan to rule brain bleed after several fainting episodes. Radiologist's report reads: "RADIOLOGY REPORT*

Clinical Data: Near-syncope while showering. Fever.

CT HEAD WITHOUT CONTRAST

Technique: Contiguous axial images were obtained from the base of
the skull through the vertex without contrast

Comparison: None.

Findings: The brain has and unremarkable appearance without
evidence for hemorrhage, acute infarction, hydrocephalus, or mass
lesion. There is no extra axial fluid collection. The skull and
paranasal sinuses are normal. Slight prominence of the CSF spaces
could represent premature atrophy. Correlate clinically.

IMPRESSION: No acute intracranial findings. Question mild
premature atrophy."

I am scared to death and have to wait three weeks to see a neurologist. What could all this mean. I do not have any symptoms or a family history of neuro diseases. Please help!

Brief Answer:
NEAR SYNCOPE WHILE SHOWERING needs investigations

Detailed Answer:
Dear Ma,am
Hello
Wishing you a speedy recovery .
As per clinical history given by you , you had one episode of near SYNCOPE during shower . & you are asymptomatic now .
I have gone through your CT SCAN report . NOTHING MUCH TO WORRY ABOUT AS CT BRAIN IS NORMAL mild premature atrophy is a normal phenomenon .
NO ACTIVE NEUROLOGICAL TREATMENT NEEDED FOR THIS .
BUT i would suggest you to get yourself further investigated to rule out any cardiac/metabolic cause for syncope.

NO NEED TO BE SCARED . just take precautions like DRIVING CAREFULLY /AVOIDING ADVENTURE SPORTS etc.

Thanx

Dear Dr. XXXXXXX

Thank you for your reply. Unfortunately, I have to disagree with you that premature brain atrophy is a normal finding. This suggests that my brain is decreasing in size prematurely, as I am sure I do not need to explain to you, and could be a sign of diseases ranging from premature dementia to Huntington's disease. That is why my general practitioner referred me to a neurologist. I am very concerned about having one of these diseases. How would you explain that "mild premature brain atrophy" is a normal finding? My general practitioner spoke to the radiologist who read my CT scan and he said it "definitely was not normal for a 34-year-old." I would like some more details. Thanks.

Sincerely,

Brief Answer:
premature mild atrophy clinical correlation needed

Detailed Answer:
Dear Ma,am
Hello
YOUR concern about mild premature atrophy is genuine
Anything seen/reported on any radiological investigations needs clarification.
WE AS NEUROSURGEON/CLINICIAN see any of these findings along with clinical correlations . i.e. if your radiological findings are correlated with your definite clinical symptomatology , then it carries any significance .
SECONDLY any finding which is subjective on CT SCAN , may be interpreted differently by different clinicians . May be that your normal brain anatomy is like that .
THIRDLY fainting/syncopal attack may occur with normal brain .

My advice would be to consult your neurologist for checking about any clinical correlation with your radiological findings . You can also get an MRI BRAIN to confirm any significant changes in brain .

Thanx

Dear Dr. XXXXXXX

Thank you for your follow-up. I have no clinical symptoms of neurological disease such as memory loss, confusion, difficulty with speech or movement, etc. I have had multiple neurological exams in the past few months by two general practice doctors and one by the physician's assistant in the emergency room who ordered the CT scan and passed all of them. I am thinking it is likely my brain is simply built this way, but I still intend to see the neurologist. I am just very scared in the meantime that I have some horrible disease.

Sincerely,

Rivkah

Brief Answer:
neurologically normal DO NOT WORRY

Detailed Answer:
DEAR MA,AM
Hello
Wishing you a speedy recovery
Sorry for unavoidable delay due to some emergencies .
I am happy that your all neurological examinations were normal .
In my opinion ,you should not worry much . You should not be anxious if all your clinical exams are ok .
Get yourself checked by a neurologist ,along with CT SCAN images .
In the meantime you can also try some anxiolytics for symptomatic relief .

Thanx
