Brief Answer:
Investigations advised

Detailed Answer:
Hello dear, sorry for the delay in response,

In my opinion get a peripheral blood film, Hb electrophoresis, reticulocyte count and Cardiology consultation at the earliest.

She has a massive splenic enlargement and thus cannot be taken lightly along with her hemoglobin and portal hypertension.

Also, she should start taking aspirin as well after consulting her doctor and get her blood pressure checked as well.

You can discuss this anytime on this forum,
Regards
