Question : After trying everything, I had spine fusion at L4-5 last October because of ongoing pain. Still having ongoing pain, and referred to pain management, where they did an epidural steroid injection and a facet rhizotomy, and increased my meds (Nabumetone and Gabapentin) with no relief. Just got a CT scan and got these results:
L1-2 - slight disc space narrowing and osteopathic spurring. Mild facet joint changes present
L2-3 - no significant disc space narrowing, spurring, or other acute changes
L3-4 - mild to moderate facet joint changes, mild diffuse disc bulging present. Ligament up flavum hyper trophy may be present. There does appear to be slight narrowing of the neuroforamina bilaterally.
L4-5 - interval anterior fusion present. Interval laminectomy, with screws present from posterior approach. This is causing artifact. There is minimal anterior spondylolisthesis. Post surgical changes are present in the posterior soft tissues.
L5-S1 - slight retrolisthesis. Mild facet joint changes.
Spurring of the SI joints with adjacent bony sclerosis.

Just trying to figure out what this means. I've had every non-surgical treatment available, and am on XXXXXXX meds. I'm a secretary and sitting all day is killing me.

Brief Answer:
You have multiple spine changes that are causing so much pain.

Detailed Answer:
Dr. Hanif warmly Welcomes you to my virtual clinic HCM!
I have carefully worked through your case, and can realize your health worries. Being your physician, I will take care all of your medical concerns.

First of all I feel sorry for the situation and pains you are in. The CT scan result you shared is remarkably positive for some derangements and spine changes that are disturbing you this much.

Coming from first lumbar vertebra to the last lumbar and the sacral bone in the column, there are multiple bony lesions and degenerative changes in the vertebrae that are leading to this pain.

*They show there is joints disease-the joints which are present between the facets of the vertebrae.
*There is intervertebral space narrowing and it causes pain in a lot of people and is a major issue of modern sedentary lifestyle.
*There are bony spines (new bone growths) that pinches on the nerves and cause pain.
*There is some intervertebral disc swelling and ligamentum Flavum (a structure in there) swelling.
*There is slight change in position of the joint between last lumbar and the first sacral vertebra--This is not a true Dislocation of the joint, but a milder luxation is present.

What we can give you medically you are already on. I want to add Lumbosacral Corset for you in office hours. When you come home apply Thermacare HeatWraps around your Lumbar and sacral region. It is all needed for you right now.

Keep hydrated yourself with a judicial amount of water intake. Vegetables, fruits and salads are good for you. Take plenty of dietary fiber. Citrucel is a supplemental fiber and good for you. Add a multivitamin a day too.

If you feel nothing works for you anymore, you may be in need for multiple surgeries for the multiple issues. Try to avoid that situation.

Have a wonderful time ahead. Hope I answered all of your questions. Let me know if you have any further clinical concerns or doubts. I will be more than happy to get back to you in no time. If satisfied here, please give a generous five star rating and don't forget to close the discussion.
For any disease you have, a timely follow-up after sometime regarding the treatment efficacy and to see any need to change the treatment options is very necessary. Keep a follow up some days after this visit session ends.

Regards!
