Question: I had a recent chest CT scan with contrast for asthma issue. The radiologist report included a notation of some periportal lymph nodes mildly enlarged in size and significance uncertain. What would be a common cause for such a finding please?

1. Lymph nodes in the periportal region mean the area near the gall bladder where the portal vein and common bile duct pass together.

2. Lymph nodes in this area are not uncommon. The cause for such lymph nodes includes benign and malignant conditions.

3. The first step is to evaluate any malignancies from the liver, gall bladder and pancreas. If no cancer is found then a metastatic cause is to be ruled out. The benign causes which can give this appearance includes granulomatous lymph nodes and lipogranulomatosis. A guided FNAC test is recommended to know the cause and treat it.

I have included a copy of my CT scan report for your opinion please. My primary care physician does not seem to be concerned with the lymph nodes findings but I would appreciate any additional thoughts as to if it may be anything that would merit further investigation. Thanks so much.

1. I suggest an ultrasound scan of abdomen to have a better look at the gall stone and periportal lymph nodes.

2. Size of the lymph nodes is important and if they are more than 10 mm in size then it requires detailed evaluation.

3. It is important to get a blood work including complete blood count. Any symptoms like fever, weight loss or tiredness requires attention.
