                                    Brief Answer:
CAG is advisable .
Detailed Answer:
Hello, Sir.
I can certainly understand your concern.
As per your history, the next step in your case is to undergo coronary angiography.(CAG)
This will accurately reflect any obstruction in blood flow of coronary arteries.
Based on the results, coronary stenting can be planned accordingly.
TMT positive cases should be analysed as above.
Diastolic dysfunction can improve with medications or stenting.
You need to consult an expert Cardiologist for the above .
Post your further queries if any.
Thank you.

Thanks for the reply. During TMT I achieved a heart rate of 155 (92% of maximum) and workload of 8.7 METS without any discomfort. I don't have any prior history or symptoms. Please review and advice.

                                    Brief Answer:
CAG is more informative and reliable.
Detailed Answer:
Hello, Sir.
TMT positive for ischemia always mandates CAG as a further investigation even in the absence of symptoms or appropriate parameters attained on TMT .
Ischemia changes will be reflected on the ECG during the exercise.
This might not always cause distress.
Likewise 8.5 METS seems adequate along with the heart rate of 155.
Post your further queries if any.
Thank you.
