This looks like an EKG of an acute myocardial infarction. With the age you provided, this diagnosis is highly unlikely. I am assuming this is an EKG of an older adult who experienced chest pain. If this is the case, or if this is your case, however, immediate medical attention should be sought in the nearest emergency room. This should be carefully differential diagnosed with an acute inflammation of the heart lining (pericardium), but the downward movement of the ST segment in the leads I and aVL make this diagnose unlikely. I would recommend a test for troponin and creatine phospho-kinase myocardium brain (CK-MB) levels and a plain chest X-ray, as soon as possible. Based on the results, further tests or procedures may be warranted, such as coronary angiogram. Just to be thorough, acute coronary syndromes in healthy and young people happen only to those who have anatomical abnormalities of the coronary arteries or to those who use drugs such as cocaine, which has a high profile of thrombi generation.
