What Does My EKG Result Report Indicate?

Question: Hello,

I purchased a 1-lead EKG monitor to attempt to capture some of my symptoms and actually produce an EKG to show a doctor. If you are not aware, this is the "InstantCheck" model where both thumbs are pressed on electrodes. I do not know which, if any, specific ECG lead these mimic. As you can probably see, the ECG itself is sensitive to artifacts from movement, etc.

Getting right to my questions, I am going to upload two images, with two separate questions. In the first one, you will see a wide wavy lines between two beat in the top-right, and then again in the bottom-right (a very large one). Is this a PVC? Is this an R-on-T PVC?

My the next image is a different EKG taken, and at the bottom right you can see that the T-waves are flat, and VERY wide. Other T-waves look smaller, narrower, and taller (perhaps normal). I suppose I am wondering if the wide T-wave pattern in several beats, but not all, is hazardous?

Just for your reference, I have seen two cardiologists in person. But I plan to go back to one of them, or see a third, because my heart/heart-beat just does not feel right (ongoing problems, so not going to ER unless something extreme would happen).

(disregard both time-stamp on image (not correct), and note that any data in the top box is for the beats between the black lines, and is computer-generated).

I'm waiting for your data!

Detailed Answer:
Hi and thank you for asking!
I read your query very carefully and I'm here to assist you with my advice.

Can you upload the monitor data image? This way I will evaluate them for you and will be able to give my opinion.

Moreover I would like to know:
- Have you done any cardiac examinations (like echo, stress test or holter monitoring)?
- If yes can you upload the examinations results for me?

I'm waiting for your data!
Meanwhile, wish you health!

Hello, i tried uploading again.

As fir Holter results, i only know that 7 PVCs and 183 PACs were found in 24 hours. I dont have report but doc said "normal."

Stress echo was "normal." Then, two 12-lead ECGs showed T-wave abnormality, one noting inferior inverted t-waves, and biphasic t-waves. One noted long QTc, while the other did not

My presentation to Dr was indeed for PVCs..

Hi back,

After reviewing the ecg monitor image I would like to say that the first image is an artifact and shouldn't worry about. The second image shows a PACs, which isn't dangerous and shouldn't worry about.

Appart from discomfort premature beats (PACs and PVCs) are not harmful as long as your heart echo, holter monitoring and stress test were normal. You don't need to see another doctor. For the moment onlya healthy diet and lifestyle and redoing cardiac exams after 1 years including:
- ECG
- Cardiac ECHO
- Stress test
- Holter monitoring

You should stay calm and relaxed continue with a healthy life style and diet as follows:
- Eat a healthy Mediterranean diet reach in fresh vegetables, fruits and white meat, poor in diary products.
- Reduce alcohol intake to less then a glass of vine per day.
- Avoid caffeine, tea, energy and carbonated drink.
- Eat often and in small portions.
- Avoid laying down to sleep 2-3 hours after eating.
- Reduce unnecessary stress.
- Use chamomile tea.
- Use Magnesium reach multivitamins tablets.

The QTc on this ecg is normal to. However can you upload the ECG with long QTc to check it by myself?

Hope this will be of help!
Wish you health!

Hello,

To answer your last question, here is an example of an ECG I recorded where some of the individual beats have a very long QTc interval.

In this image, the beat highlighted in blue, as it shows, has a QTc of 473 ms (very long for a man). Yet, the entire strip shows 405 ms. I guess my question is if a single beat with long QTc is still a concern (because something is causing the delayed repolarization for some beats but not others), or if every single beat has to have the long QTc for it to be a problem?

Brief Answer:
Following advice.

Detailed Answer:
Hi back,

After reviewing the monitor ecg image (even the blue part) I would like to say that it serve as a orientation toolin the measurement of QT and QTC interval.

The monitor ecg is not as accurate as the standard Ecg. It would had been taken in consideration if the prolongation of the QTc would have been observed on all the recorded QRS complexes.

However, this founding shows that you should continue with healthy life style and regularly cardiac follow up (including heart monitoring).

Hope this will be of help!
Wish you health!
