Hi, I had gone through your question and understand your concerns.
Extended cerebellar tonsils below foramen magnum 1.2 cm instead of 5 cm before surgery means that your headache and dizzynes probably have another cause instead of recurrence of Chiari malformation.
During surgery there is decomprression of posterior fossa and foramen magnum, so extended tonsils should cause no symptoms.
I suggest you to consult and discuss with your treating Doctor about other causes of headache and neck pain.
Another thing you may discuss is the relationship of POTS syndrome with Chiari malformation.
In my opinion you could have moderate physical therapy, even there's POTS.
