Question: Hello Doctor , I am a 23 year old male , today I have received my cardiac MRI report and I will attach the file down below , I have had a PVC ablation one year ago and my MRI at that time showed a bit bigger LV volumes and now after one year post my ablation I want to compare the old and the new MRI findings , i will send the pictures of both reports , has the size of my LV volumes improved from the old MRI ? Thanks

Brief Answer:
Your cardiac test is perfectly normal showing marked improvements.

Detailed Answer:
Hello xxxx
Welcome back to HCM! I am glad to talk to you again!

We have already talked together one day ago, related to your cardiac tests.

As now there is a new test available, after I reviewed it carefully and compared with the previous performed tests, I would like to confirm that there exists an obvious improvement in cardiac volumes from the previous cardiac MRI.

So LVEDV from 207ml has decreased to 194.8ml and your LVESV from 101ml has turned to 82.2ml.

Your cardiac output is quite perfect 8.4ml/min and your LV ejection fraction is normal too.

So, I am glad to confirm that your last cardiac test confirms quite a normal cardiac function and structure, and not coming close to the upper limits of the normality any more.

So just relax! There is nothing to worry about any more.

Hope to have been helpful!

I am at your disposal if you have any further questions, whenever you need!

Greetings!

Dr. Iliri

Hello Dr.IIir I am really happy That I get to talk you , I was hoping you would be the one reviewing my questions today ;)
You have just made my day as I could not understand those findings , I read that the study was normal but I really wanted to know if those LV volumes sizes had improved since my PVCs have been a lot less frequent over a year now after thr ablation , so this makes me believe that if I had not gone ahead with the ablation and I was still have thousands of PVCs my heart would keep getting bigger , maybe they are not completely harmless afterall ? I just hope to maintain normal sinus rhythm in the future , do you think the risk is high for them to come back ? Since today has been very stressful day for me and I have been anxious all the time i have had a few skipped beats I felt in my chest , currently I take Bisoprolol 1 . 25 mg once in the morning , I have been taking this for almost two weeks now and what it helped me with is the feeling PVCs cause in your chest , when i get isolated ones they feel less intense and that helps me as I have anxiety and panic disorder ( I get panic attacks if I get too scared when i have a skipped heart beat ) some doctors say i dont need beta blocker do you think its a good idea to keep taking this medication ? Thank you !

Brief Answer:
I would explain as follows:

Detailed Answer:

Hi xxxxxx

I believe that there isn't any increased risks for those ectopic cardiac beats to come back again, as cardiac ablation is considered successful and has yielded beneficial results.

You just need to control your anxiety, as it may produce the feeling of cardiac palpitations. I recommend you engaging in more physical activities, such as aerobics, swimming, cycling, etc. which may help you relieve your increased anxiety.

Regarding Bisoprolol, a 1.25 mg daily dose is quite small and could not produce any adverse effects such as decreased physical performance (which is common with beta-blockers in young adults). So, it would be wise to continue that medication at least for a while and then it may be completely stopped gradually.

You need to closely discuss with the prescribing doctor when it will comes the appropriate time to stop the drug.

Thank you so very much Doctor IIiri , I really appreciate your help ! I am now trying to relax as having stress and being anxious causes me more skipped beats , I'll continue with the beta blocker for a bit until I can manage my anxiety disorder .
I think my skipped beats are very sensative to adrenalin as i get them during intercourse as well and it has kinda bothered me a lot , and I have even feared to have a normal sexual life but now i am glad my cardiac MRI is showing some improvments . Thanks a lot !

Brief Answer:
You are welcome!

Detailed Answer:
Dear XXXXXXX

I am glad to have been helpful!

Regarding intercourse, you should know that it is quite normal to have some palpitations during this activity.

It happens in all normal persons.

As your cardiac tests are normal, you have nothing to worry about.

You can try also yoga, meditation and physical activity (walking, aerobics), to help you face with your anxiety.

Hope to have been helpful!
