Question: A recent brain MRI shows innumerable foci ot T2 signal alteratio throughout the periventricular and subcortical white matter. Additional foci of T2 signal alterations are noted on the pons. What does this mean?

First possible diagnosis in your case is small artery disease of the brain, a condition where terminal small arteries of brain are narrowed causing this way very small infarcts.

This is usually a finding after uncontrolled high blood pressure, diabetes or in rare cases Vasculitis.

Another possibility of these small white matter lesions is demyelination seen in diseases such multiple sclerosis.
