Question: Good evening doctor,
I have been having pain in my left knee and was advised by my orthopedic that an MRI scan is needed.Please find attached the MRI report.Prior consulting my doctor i wanted to check with you what does the report indicate and what should be the line of treatment.I work in the merchant navy and work for periods of 4months at sea.Kindly advise.

Do you have pain while climbing stairs?

Let me start with replies to your queries:
1.Pain while climbing stairs is there but not really troubling me as such.I stay on a 4th floor flat without a lift and when i am on a vacation from my duties on a ship i climb up and down the stairs about 3 to 4 times a day.My weight is about 86kgs Height 167cms.

2.There is pain while squatting in the left knee.I would like to sit on the floor a few months back but now i refrain from doing it.

3.Cracking sounds from knee :That is not very evident.It may be occasionally.

I have been walking for more than a hours sometimes two hours about 4/5 years back but now if i walk for a few days i feel the pain and than stop.

The doctor i have spoken to says there are two options:
1.To go in for surgery which would take about one month to get back to your normal activity I presume it is the type of surgery you have advised "Arthroscopic surgical treatment".I am not sure whether he has advocated the "open" surgery.
2.The second option is a temporary measure that is to inject gel into the area affected to reduce the friction and wearing away.But this will last for some time and may be required to be done every one or two years.
As i am required to join a ship i am in a dilemma to whether i can do it after i come back after 4 months or so.
Also want to know what are the effects of both the treatments.
Are there any side effects,any restrictions on activity (as i like to be busy physically also).
I am fairly busy physically when i come home like carrying weights about 5 kgs from the markets 10 minutes away.
Also would like to know if medication will help in solving the issue or reduce the pain.
For your information about a year back i was taking calalog,Jocal c,and d3 tablets prescribed by another specialist.
Now the doctor has prescribed Rosiflex,Celadrin,Flexlirad and pan 20.
Appreciate your views.

You can get the gel injection which will give relief for a few months, but this won't solve the pain in squatting. This is because if the meniscus tear which needs to be surgically treated.

Hence it is best to get the surgery done otherwise arthritis can occur sooner. Side effects of injection include infection, temporary restricted movements, recurrence if symptoms after 4-6 months. With surgery, there is about 6 months rehabilitation time where you can't bear complete weight, possibility of damage to the joint iatrogenically, but less chance of infection as compared to regular open surgery.
