default
Posted on Tue, 28 Mar 2017
Question: During a recent 3 week hospitalization, on an mri I had two incidental findings 1. several hepatic lesions one measuring 1.9cm x 1.2cm x 1.8cm they recommended follow-up mri to exclude other etiologies besides hemangioma . 2. A 1cm nodule and a 9mm nodule on my one of my adrenal glands and recommend adrenal protocol ct as metastases or additional malignancy cannot be excluded. I have a family history of liver cancer, and 2 siblings on dialysis and one sibling with kidney cancer of both kidneys. My doctor says no follow-up this is nothing to be worried about. Should I be concerned or worried?

Detailed Answer:
Hi,

Thanks for your post on HCM

I do understand your pain and discomfort

“As” per your clinical history is concerned which I checked in detail. You want to know whether you should be worried or not in my own opinion I may say imaging tests are not enough to arrive at a point and that is not final. In adrenal glands there are also benign growths present. But to ruled out major disease we have to ruled out cancer first as per your family history.so you can discuss with your treating doctor regarding few laboratory investigations like :-

1)Serum glucose

2)Serum cortisol

3)Serum adrenal androgen

4)Urine adrenal hormone

5)Urine vanillylmandelic acid (VMA)

6)Urine homovanillic acid (HVA) levels

Secondly I may say that in both the tumor size is less than 1 cm so we can not say definitely that it is adrenal cancer so to suspect as adrenal cancer it should be 4 cm or more in irregular shape and Intralesional calcification must be present, so wait and see lab reports only.

Hope that helps
