Question: Hello Dr. the finding in MRI were minimal subarticular marrow edema showing possibility of sesamoiditis,thinning of articulate cartilage,osteophyte formation and narrowing of joint space with degenerative cystic change in metatarsophalangeal joint of the big toe.And slight hallux vagus deformity.My question is:does articulate cartilage regenerate?How to dissolve the osteophytes?How long will the marrow edema take to heal?And how to go about all these.I am a fitness trainer and need to remain fully functional.Thank you

Hello,

MRI findings suggests degenerative changes.These are generally age related changes but findings are early in your case.
There are also mild signs of inflammation in the form of inflammation in sasamoid bone.
Thinning of articular cartilage,joint space narrowing and osteophytes are all related to degenerative changes.

Articular cartilage is not regenerated as it is not supplied by blood vessels(avascular structure)but process of degeneration can be slowed down by medicines like hyaluronidase supplementation.

Osteophytes can't be dissolved.It can only be excised surgically if it is causing symptoms like compression to surrounding structures etc.

Marrow edema is most likely linked to inflammation.You may need steroid injection locally.It may take 4- 6 weeks to subside edema after control of inflammation.

You may need physiotherapy and clinical correlation.
You may need also need routine investigations including uric acid estimation.
