Question: Hello my doctor diganosied me with peribroncial cuffing on a chest x ray he believes it was caused my asthma I've been having mild chest pain all around with slighly more difficult breathing for about six weeks. He believes my childhood asathma came back I've been treated with symbacort singular and lorasitine and b12 for four days without a huge improvement. My EKG and ecg along with echocardiogram came back normal the echo was done 10 says before fluid peribronchial cuffing was found on x ray. What are the odds I'm having a heart problem?

I am Dr.Muhammad Ahmad. I am an trainee in entensive cardiogy myself.I have read your question completely, i understand your concern and will try to help you in best way possible.

According to you history there is almost no danger of you having shortness of breath due to a heart issue here are a few reasons.

1) You have no history of heart issues.
2) Shortneas of breayh due to heart is usually aggravated when one is lying down(orthopnoea)
3) In your age heart issues are rare(unless by birth)
4) In breathing issues due to heart there are crepts sounds on chest he must have lsitened to your chest and he ruled out heart means you didnt have any added sounds.
5) ECG mostly shows changes in heart issues which yours didnt.
6) Echo was normal which almost rules out any heart issues.
7) There was finding in the lungs on Xray which is not usually there in heart issues.

I hope this answered your question, a

My father had arterial seclerosis and died at 44 of a heroine overdose. He had a bad lifestyle though. I'm 6ft 175 don't smoke and hardly drink although I had second hand smoke for 18 years. It is a little worse when I lay down but not like choking just like I need to use a little more Stregth but not really gasping of air. I also have a hard time with sexual activity like if I go to masterbate as soon as I start to get an erection I get shakey and fall to the floor. I'm not a very anxious person I took norco two times a day 500 mg from ages 14-18 been clean for two and a half years. So my concerns are can this be an issue with my arteries? If so what blood tests can my doctor run to rule it out. Also I have a vitimin b12 defciency reading of 353. And what is your opinion of my doctors diganosis of peribronchial cuffing being caused by reoccurring childhood asthma? Is this likely or with all the other symptoms it my be somthing else?

Brief Answer:
It doesn't seem to be heart

Detailed Answer:
Hello ,

Thanks for writing back,

What ever risk factor you have still arterial disease is very less likely,
Here being a cardiologist myself I would like to explain things to you, breathing issues due to heart have a long sequence of happening, first a person suffers a heart attack then his heart gets weaker and weaker then he starts to feel effects on his breathing since you never got a heart attack and your ECG and echo (which almost always shows defect in the heart if there is any issues) is normal you do not have to worry about heart issues.

Yes i agree with your doctor , in light of your history and present symptoms plus the Xray findings peribronchial cuffing seems to be the best possible answer.

I would suggest you to follow the treatment suggested by your doctor and i am quite positive that you will have great improvement in a couple of days. And yes passive smoking does aggravate the problem so avoid it.

Regards.

So the peri broncial cuffing seems to be do to asthma? Also is there any other causes that can be causeing this issue? This supposed asthma just came out of no where and this peribronchial cuffing did not affect my peek flow score of 525 and my peek flow when I did not have peribronchial cuffing was 500. Can this be? Also I don't smoke. And what about my issues with masterbation? Like feeling like I'm gonna fall over as soon as I start to get an erection? Thank you for your help.

Also if this is asthma induced peribronchial cuffing, how long will it take with treatment to clear up, the discomfert associated with the peribronchial cuffing gets worse when I eat, is that normal?

Brief Answer:
It will get better with time.

Detailed Answer:
Hello thanks for writing back,

Vitamin b 12 defficiency can play a part too it causes anaemia which can cause both shortness of breath and chest tightness... Lung function tests sometimes dont change much in mild forms like if there was a difference of 25 it isnt much significant... yes cuffing can be there is pulmonary function test is this way.

every one reacts sexual drive in a different way some feel palpitation some feel raised heart rate some feel drowsy and some get mood swings I would suggest you not to be much concerned about this ...may be you had this already but it wasnt significant but now as you have B12 deficiency and possibly anemia with breath issues and anxiety thats why you are feeling it more.

According to you history and symptoms asthma is the most probable cause... it can get aggravated by sand or allergen inhallation or any chest infection even if mild...with proper medication it can subside in a week or so.

I would suggest you not to.panic everything should be fine soon.

regards.

Dr.Ahmad.
