What Does My Biopsies Result Indicate?

Question: Recently had a whole body check due to melanoma history. 9 biopsies were take . Drs office called me back for a further shave biopsy of one . - diagnosis " pre cancerous " i picked up the pathology report at the same time and read it when I got home. Of the 9 , four ( including the one I had an extra shave done on ) we diagnoses as Dysplastic Nevus. The otehr five specifically said the words benign.

The one that had further removal was described as :

Junctional melanocytic nevus with architectutural disorder and moderate cytologic atypia ( Dysplastic Nevus ) with a note saying : The lesion extends to one peripheral margin. The microscopic description says - A junctional melanocytic peroliferation of moderately atypical melanocytes arranged as nests and focally as soliatray units is present at the dermal epidermal junction .

There are three more that have the same description except instead of moderate they are mildly atypical but they all say the lesion extends to one peripheral margin.

Bearing in mind Ihave already had two melanomas and the fact that the pathologist has clearly identfied the other samples as benign , can you explain why I would have only had a further shave biopsy on the moderately atypical one.

Do you think I should seek a second opinon ? I am concerned that something may happen later on that I cant monitor now the mole has gone.

What is your opinion/advice on this please.

The three samples were .4 x .4 x .1 cm
.5 x .4 x .1 cm
.5 x. 4.x .1 cm
and were not in a visual area so scarring is not an issue

Hi

Dysplastic Nevus are unusual benign moles that may resemble melanoma.

People who have them are at increased risk of developing single or multiple melanomas.

The higher the number of these moles someone has the higher the risk of melanoma.

Those who have 10 or more have 12 times the risk of developing melanoma compared to the general population.

Most dysplastic nevi do not turn into melanoma.

Most remain stable over time.

Chance of melanoma is about ten times greater for someone with more than five dysplastic nevi than for someone who has none, and the more dysplastic nevi a person has, the greater the chance of developing melanoma.

Since your lesion extends to one peripheral margin you need further shave biopsy on the moderately atypical one so that the surgical margin is completely clear of the lesion.

You will note that I have had further shave biopsy on the moderate atypical, as as a previous 2 x melanoma patient I am already at risk regardless of how many atypical moles. My question was more reagrding the mildly atypical moles that extend to the periphial margin. Should these have further margins removed

Hi

Moderate to severe atypical moles require further excision to make sure that the surgical margin is completely clear of the lesion.

In your case it is mildly atypical moles and further margins removal is not required.
