What Does My Blood Culture Report Indicate?

Question: Hi,
We are going through very awkward situation. My 2.1 years old daughter has been suffering from fever on and off since last Monday night i.e. 22nd May. Initially she had bit of loose motion also along with fever. We controlled fever and loose motion using Meftal P, ORS and Zifi 50 mg/5ml. Loose motion has been controlled but fever continued till yesterday. It was 6th day of fever which was most of the time around 99.5 raising till 102.5. So we visited our pediatrician considering if this is a viral fever then must have gone by now. Though baby is eating and active when not in high fever.

Pediatrician asked us to carry out CBC, Typhoid, Malaria, NS1 Antigen, CRP and Blood Culture tests. Report is attached. In report HB is unexpectedly very low. To investigate it further my pedia asked to carry out few more tests like Iron Serum, RETICULOCYTE COUNT, PERIPHERAL SMEAR EXAM, TOTAL IRON BINDING CAPACITY, FERRITIN SERUM, 25HYDROXY, VITAMIN D and HEMOGLOBIN VARIANT ANALYSIS. Reports for Blood Culture and HEMOGLOBIN VARIANT ANALYSIS is pending, rest are uploaded here with my question. We have also done urine routine test which is normal. Unable to attach the report because of restriction on website.

From the report I can understand there is something related to iron deficiency. Probably my pediatric is going to explain me more. Meanwhile I expect observation from Healthcaremagic also. I would like to know following:
1. These deficiencies or abnormal values in uploaded reported are just because of improper diet or they might be like this in body since long and there might be other causes as well.
2. Are these values get tweaked due to any viral infection which can happen all of sudden or we are lucky to know these abnormalities while investigating cause of fever which started last Monday.
3. If this is possible that fever started because of these deficiencies or abnormality in iron or HB.
4. What shall be further course of treatment or investigation.
5. Why CRP is abnormally high in the report. Can it be related to stomach infections as light loose motions were there in initial couple of days.

Baby is a girl and weighs around 12 KG age 2 years. In present Pediatrician has asked to provide Taxim O Forte antibiotic syrup 5ml twice to baby and we are waiting for two pending reports.

P.S. Of course I am in touch with my pediatrician. However, any observation / suggestion based on the reports uploaded are most welcome.

I have gone carefully through your query and understand your concerns. Let me try to address the issues one by one.

1. The iron deficiency should be directly linked with diet although causes like worms need to be excluded. Vitamin D deficiency is likely to be more importantly due to lack of adequate exposure to sunlight although the supplementation in diet also plays a role.

2. I do not think that the viral infection caused the values to be tweaked. It is just that the deficiencies got detected while the tests were performed. It is not unusual for these to have otherwise been missed.

3. The fever would have not started due to these deficiencies.

4. Proper correlation is needed. The initial management would be paracetamol and tepid sponging to bring down the temperature when needed. Same about Meftal P, ORS and Zifi. If the fever lasts for more than 7 days further testing to rule out enteric fever would need to be done. The CRP level also needs to be rechecked at that point of time.

5. This too calls for a proper clinical correlation. Rise in CRP occurs due to inflammation or infection. It needs to be evaluated if there could be a focus of inflammation being missed. The reduction in CRP levels in a follow up test would be reassuring even before the CRP level is back to the normal range.

Hi,

Just got a call from pathology and as per them Blood Culture is positive and it is "Gram positive cocci". What does this mean and what shall be the future course of action.

Thanks for writing back. The report means that an infection has been detected as evident from the growth in blood culture. She is already on Zify, an antibiotic. The sensitivity report (that should come in a day) would confirm if the bacteria are responsible to the antibiotic being given or whether a different antibiotic would need to be added. Treatment with the sensitive antibiotics would clear the infection and resolve the problems.

In addition to the above, iron and vitamin D supplementation would be needed.

Thanks for your prompt reply. Just now I have got the final report of Blood Culture. Can you please see and suggest. In present she is on antibiotic since Sunday night Taxim O Forte 5 ml twice. From the same time there is no fever. Blood samples were given before starting antibiotic. If any thing is missing in investigation.

I would also like to know how this infection came into blood. We have following entire vaccination schedule.

Blood culture report attached.

Also attaching report for Hemoglobin Variance analysis. This report along with other reports which are already uploaded may help you to analyze low hemoglobin counts.
