Brief Answer:
Dear sir, Here is the list of things to be done to keep u healthy & happy

Detailed Answer:
Dear sir, Greetings .. i understand your complaints and concern for your food habits... first you need to get your lipid profile, microalbuminuria and HbA1c checked. your medications are sufficient for time being if you are committed for lifestyle changes. As you understood correctly lifestyle changes (diet + exercise) are more important in diabetic and hypertensive cases along with the medicines which u r taking. It is good to note that you are consuming medicines for BP, sugar and high cholesterol levels too.
Please prepare yourself a diet chart based on your food habits.first stay away from all the fast / processed / and fried food stuffs .. divide your meal in to 4-5 parts per day whenever you feel hungry.. add more of fiber rich food stuffs like green leafy vegetables, raw fruits.. (not juices), eat very less of carbohydrates (rice) avoid sugar completely.. take Millets and breads prepared out of it like ragi.. ragi is a very good source of calcium too...do take more of proteins ... like egg white... say no to oils and cholesterol rich food stuffs ... you can take fish (boiled) but chicken (without skin) rarely...sir, based on this you prepare your diet chart and strictly follow it ..Do exercise .. walk at least half an hour to one hour a day ... try and practice yoga and pranayama ... some aasanas are very easy to practice and also beneficial.
Quit smoking if you are a smoker and reduce your alcohol intake. A glass for beer or red wine once in a week is good as red wine has more of antioxidants. But make sure you are not crossing 1/2 glasses. Moreover, beet root juice (inorganic nitrate), olive leaves, garlic, turmeric, tamarind also have proven benefits.
If u can follow these (have to!!) you can definitely see difference yourself by the time you have your next blood investigation report in two months ... if you have any other query please feel free to ask me .. attach the report also ...ALL THE BEST & take care
