Hi back,

After reviewing your echo my opinion is that you still have moderate aortic regurgitation with a mild aortic stenosis.

If you didn't have experienced episodes of shortness of breath, syncope or chest pain for the moment you should continue with actual treatment and follow up regime. Since your LV function is normal and LV end diastolic diameter is 5,9 cm you don't need any change to treatment strategy.

The Regurgitation velocity is 470cm/sec and PG is 88.7mmHg are these parameter are normal. Also i regularly have 2km brisk walk and no shortness of breath episode. Further advice me the follow up.

Let me explain these findings to you. As I said you have a moderated aortic regurgitation based on dopler parameters especially Aortic Regurgitation (AR) PHT which is 264m/s. This is one of the most important doppler parameters to determine the severity of aortic regurgitation and a PHT of 264 m/s is indicative for a moderate aortic regurgitation.

The other data you mentioned aren't normal and are in correlation with the PHT and other datas that shows a moderate aortic regurgitations.

After reviewing the data of December echo I would like to say that on December the AR velocity was 429 cm/s and PG was 73 mmHg. On the actual echo there's an slightly elevation of AR velocity of 470 cm/s and of PG 88.7 mmHg.

This change are more likely related to the fact that you have an athlete heart and there's a slight remodelation of LV from athletic training. Also the progression of aortic reguqrgitation play a minor role on the alteration of this parameters.

However you are still with moderate aortic stenosis and you don't need surgery for the moment. The only recommended treatment for the moment are:
- medical treatment with a ACE-i like ramipril to reduce LV remodeling from aortic regurgitation jet. - you should continue with physical activities as you are already doing
- and you should continue with regular follow up with echocardiogram every sixth months.

some report says mild aortic stenosis and some says no AS.PLEASE CLARIFY THE ACTUAL DISEASE AND ALSO IS THERE ANY TRQUIREMENT OF STATINS AND BLOOD THINNER

This change on evaluation are typical for mild aortic stenosis because on mild aortic stenosis there's a slight increase of transaortic pressure.

On actual echo the pressure is wothin upper limits and this is why it's considered as normal, for the moment you have only modetrate AR. However this is a parameter that should be monitored with echo every sixth months.

Statin use and blood thinners are of little help on your case (for the treatment of AR) and my opinion is that you don't have to take them at this stage.
