Brief Answer:
May be an issue with heart or the blood itself.

Detailed Answer:
Thanks for the consultation at my virtual clinic HCM!
I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

We usually advise this test to see the ratio of concentrations of BUN and Creatinine in blood and to see what possibly is causing it to change.

10:1 to 20:1 is normal value in most laboratories and you have High value of 43:1 that means you have lower than normal blood supplied the kidneys (both in terms of flow and amount).

Reason for high ratio may be:-
*Heart disease (congestive heart failure)
*Related to the blood (fluid and electrolyte imbalances due to dehydration).
*High amount of serum proteins (cause may be gastrointestinal bleeding or high protein intake).

Your blood sugar control has not been so good for the past 4 months or so as is depicted in your HbA1c levels that are higher. But with proper physical activity and diet control you may easily get better levels soon.

As muscle mass decreases the levels of creatinine also diminish. Your levels are not that low. And also raised levels are more of a concerns than low Creatinine levels.

Hope I answered all of your questions. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with expert opinion. Please don't forget to give Five Star rating to help me assist my patients in a better way; and, please don't forget to 'Close This Discussion'.
For any disease you have, a timely follow-up is very necessary regarding the treatment efficacy and to see if there is a need to change the treatment options. So keep a follow up to see how you doing (Always write down my name in the start of your questions at HCM & I will be with you in no time).
