                                    Brief Answer:
No pacemaker needed based on holter report . Regards
Detailed Answer:
Hello XXXX, I went through your reports . I would note down my observations as follows .
1) The blood investigation are normal .except your CBC which shows increased wbc counts which suggest some infection or inflammation and platelets on lower side which is unexplained. Do get that evaluated by your physician
2) The holter doesn't show any junctional bradycardia and there is nothing in the holter to suggest the need for pacemaker at present whether temporary or permanent.
3) The discharge summary of the admission at night, writes your ecg as junctional bradycardia, however there is no ecg where I can confirm that diagnosis.
Now in a 29 year old healthy male there should not be junctional rhythm.
One possibility is any infection which cross reacted with heart causing transient arrythmia ( myocarditis) , but echo is normal. .
A cardiac MRI will rule out any structural cardiac problem ( considering 2 D Echo is normal ) including any inflammation
Presently no evaluation needed . However if symptoms persists long monitoring with loop recorder just in case we are missing something. Also report to your doctor if any similar symptoms, palpitation, dizziness, breathlessness occurs.
However I would reassure you that no pacemaker needed at present considering the holter report . The maximum pause is 1.36 sec , pacemaker is recommended if pause generally is longer than 3 sec . However thing to make us be cautious in future is even 1.36 cm pause shouldn't occur.
Let me know if any clarification needed.
Regards Dr Priyank Mody
