Brief Answer:
Suggestions

Detailed Answer:
I will suggest starting Tab metformin 500 mg once a day. That will take care of your sugars.
However, I would suggest meeting a nephrologist in XXXXXXX for kidney issues.
Take a diabetic diet. avoid sugar, oily food and potatoes or rice.
Once sugar is well controlled the kidney may improve but that is not always seen to happen. So please meet a nephrologist as soon as possible.
Best of luck. Dr Mittal
