Brief Answer:
No alcohol / repeat test /

Detailed Answer:
Hi friend
Welcome to Health Care Magic

Level of Triglyceride 500 mg or above is considered VERY HIGH
Normal = less than 150 / Borderline = 150 to 199 / High = 200 to 499

You have 2 major causes – Alcohol AND Diabetes
Other common causes are – High Carb Diet / failure to fast 12 hours (preferably 14 hours) for the test / Obesity / Thyroid insufficiency / some drugs like Beta-blockers or Diuretics… you are not on any one of them / It is often genetic…

STOP ALCOHOL IMMEDIATELY / COMPLETELY / AND PERMANANTLY
REPEAT THE TEST - FASTING 14HOURS (WATER ALLOWED)
Strict control of Diabetes is in need…

Your BMI is 21.4 – Normal

Eat fewer calories / Limit natural sugars:
Eat small, frequent meals and do not skip meals
Avoid late-night snacking
Add omega-3 fats in your diet
– fatty fish (like mackerel, salmon, sardines, tuna) twice or more in a week
– soy foods, canola oil, flax seeds and walnuts / limit potatoes, yams, beans, corn and peas
Do regular physical activity – at least half an hour a day / 5 days in a week

Consult your treating Physician / May add drugs - fibrates and/or statin

Good luck
God bless you

Sir
very many thanks for your valuable advise. will like to submit further that I do smoke. so that also has to be stopped/ restricted.

also on 25/4/2014 I was operated for hernia (both side) & got my gallbladder removed (as a stone 9mm was found in it during ultrasound).

regarding alcohol I used to take IMFL. can I take scotch or other good quality occasionally?

one more question will like to seek your advise. for the last almost 2 years I am facing erectile dysfunction. consulted a doctor and got the blood test done. as per the report/ doctor I was ok but due to weakness (because of diabetes) this is happening. improved diet but to no avail. can you suggest some medicines?

waiting for your reply Sir.

Brief Answer:
NO TOBACCO & ALCOHOL / Viagra helps...

Detailed Answer:
Hi

SMOKING LIKE ALCOHOL IS TO BE STOPPED IMMEDIATELY, COMPLETELY & PERMANANTLY
NO ALCOHOL in any form… It damages the liver, heart, stomach, brain etc… especially because you have more than 2 risk factors already,

Diabetes, tobacco and alcohol - is a deadly combination – great risk

Nothing to do with the past surgeries… it only means you are OK till now – fit enough to undergo the operations…

For erectile Dysfunction, the ideal option is to consult an XXXXXXX (Urologist). He may do a few basic investigations and advise appropriately. In the meantime, you may benefit from sildenafil (Viagra) or another from similar class…

Brief Answer:
Life style chages AND drugs

Detailed Answer:
Hi

Get the test repeated with full 14-hour fast and abstinence, as soon as possible
If the value is still elevated, a drug is necessary.

As already mentioned in the first response, fibrate or statin will be started by your physician…
Fibrates are preferred here because of disproportionate triglyceride elevation…

Fenofibrate 160 mg may be started first / a statin like atorvastatin (10 to 80 mg) may be added later.
It is usually given once / in the night.
Some give a combination tablet straight away – there are several preparations in the market like Zivast-F, Atorlip-F and so on and on and on…
Prescription services are discouraged in a public service like this / Discuss with your doctor and he will give appropriate prescription
