What Does My Pelvic Sonography Test Report Indicate?

Question: Hi, I had visited gynaecologist on 13th July as I am vomiting during periods (first two days) since last 5 to 6 months. She ask me to go for Pelvic Sconography. I have done the same and the result shows uterus is anteverted, empty and meausres 7.8 cmx5.8 cmx4.1 cm. endometrial thickness 5.4 mm. Cervix and cervical canal appear normal.

THere are multiple uterine fibroids
1) 3.7 cmx3.2 cm size subserosal fibroid
2) 2.1 cm x1.8 cm size subendometrial fibroid
3) 8 mm x7mm size fibroid in posterior wall

Right ovary measure 4.7 am x 3.6 cm and show 3.7 am x 2.8 cm size hemorrhagic cyst.
Left ovary measure 2.6 cm x 1.1 cm
Both ovaries are normal in size. No mass lesion detected in connection wiht the ovaries.

Minimal free fluid in cul de sac.

Conclusion : Multiple uterine fibroids. Right ovarian small hemorrhagic cyst.

Please let me know what i should do. I am getting married in August end. Is there anything to worry?? Kindly reply at the earliest.

Thanks for writing to us with your health concern.
I have gone through your question in detail.
The first concern here is multiple uterine fibroids.
Fibroids are small tumours made up of the muscle of the uterus ( womb ).
In your case, they are very small in size, and situated in such a position that they will mostly not give rise to any problems in conceiving.
The haemorrhagic cyst in the ovary needs to be followed up.
It can be due to bleeding inside normal egg, or could be due to endometriosis.
You can follow up the cyst after one month.
If it does not dissolve, or increases in size, you need medication to treat it.
If medications do not work, rarely surgery maybe needed.
It is a rare possibility, that the ovary might be compromised, and then fertility maybe impacted.
But do not worry much right now.
I will have a better idea if you can upload the ultrasound scan images with report.
I would like to go through it in detail before commenting further.

Nothing to be done right now, repeat scan after 3 months.

Detailed Answer:
Okay, now let me explain in detail.
I have gone through your report.
Fibroids , as I said, are tumours of the smooth muscle of the uterus.
They can occur in 3 positions, on the exterior surface of the uterus, inside the wall of the uterus, or on the inner lining of the uterus.
You have 1 in each position.
The fibroid that is subendometrial, is in the inner part of the uterus, can cause some issues.
It can lead to increased menstrual bleeding, pain during periods or without periods and at times, can lead to difficulty in carrying a pregnancy to term.
If you experience any of these complaints, then removal maybe tried or medication maybe tried for it.
Right now, you need to do nothing for it.
You can try progesterone only pills for 3 months, they can shrink a fibroid, and can also have a beneficial effect on the haemorrhagic cyst.
Haemorrhagic cysts shrink on their own within 2 menstrual cycles, so nothing needs to be done for that.
you can have a repeat sonogram after 3 months to compare the situation.
