Question: Can MRCP tell the difference between pancreatic psuedocyst vs. IPMN vs. MCN ? And secondly to what degree of confidence should I rely on radiologist? For example I've read that psuedocyst scan look similar to MCNs. Thank you.

Thanks for writing in to us.

Pancreas cysts require detailed investigations for confirmation and MRCP helps to a great extent in differentiating cysts in the pancreas based on imaging characteristics.

IPMN is a tumour which always connects to the duct and MCN is the one which will not connect to any duct. If this feature is clear on MRCP then it can help in making a suggestion of IPMN vs MCN.

A pseudocyst pancreas is more of a clinical correlation and prior knowledge about the patient symptoms usually helps in making pseudocyst as a possible diagnosis.

There has been research which has found that in 95 percent of cases a MCN is the likely diagnosis when a such a cystic lesion is found in the tail of the pancreas in a female .

To the best of my knowledge, in a recent study the accuracy in making a confident diagnosis of IPMN is about 73 percent.
