What Does My Semen Analysis Test Report Indicate?

Question: Iam a 31 year old male and my wife is 28. As we did not concieve after 6 months of attempts we consulted a gynec in May 2014.
I consulted a urologist too and was detected with vericocele grade 3 on the left side.
The semen analysis done in XXXXXXX 2014 had the following parameters.

Volume: 1.8 ml

Alkaline, grey white
Liquefaction by 30 mnts
Count : 63 million / ml
Total count : 113.4 ml
Excellent forward : 10 %
Good forward : 15 %
Sluggish forward: 20 %
Non progressive : 10 %
non- motile : 45 %
Abnormal head : 8 %
abnorml neck and tail : 2 % each.
My wife underwent couple of scans and everything was found normal.
With the assistance of HCG injection she conceived, however miscarried with in one month. XXXXXXX B-HCG was 103.
Soon I had to go abroad without my wife.
I took some homeopathic medicines for vericocele during this time.
Now I have returned home. During the first cycle my wife conceived naturally.
However within abt 2 weeks she miscarried. Her XXXXXXX B- HCG level was 23 .

Now the gynec has suggested for some blood tests after the end of next menustrual cycle to investigate the causes of 2 back to back loses.

Pls advice on my below queries,
1. Is the above semen analysis normal.
2. What can be the possible causes the miscarriages
3.What are the tests we should undergo further ?

Hi,

I read your query and I understand your concerns and saw your semen analysis report.

Following is my reply:

1) Your semen analysis report is normal and does not seem to be the reason for both miscarriages.

2) Possible causes of such miscarriages are generally chromosomal abnormalities. These chromosomal abnormalities make baby non viable and they die off.

3) Best option to prevent such miscarriages would be to undergo preimplantation genetic screening ( PGS) after doing IVF / ICSI ( also called test tube baby) procedure.

4) PGS test will help to identify normal embryo and transfer the same to you.

Dear doctor,
Many thanks for your valuable suggestions.

Currently we are undergoing ayurvedic medication to improve chances of successful pregnancy.
The gynecologist has asked us to undergo some tests after my wife's next cycle to detect the possible reasons for miscarriage.
Pls advice what tests we should undergo?

I dont see any need for tests. This appears to be case of chromosomal abnormality and no tests can identify it now.

PGS is best option.

Ayurvedic treatment would be waste of time.
