Brief Answer:
See an Urologist for treatment.

Detailed Answer:
Hello XXXXX and welcome to HCM.
As an Urologist, let me advise you, that, for a proper opinion on fertility status, various other factors also need to be considered.
After proper examination by an Urologist, blood sugar, male hormones like FSH,LH,PRL,testosterone, and an ultrasound doppler scan of scrotum, need to be known, to give an expert opinion on male fertility status.
If your semen test is done after avoiding sex for 2-3 days, prior to semen test, then let me give my opinion.
A minimum of 20 million sperms/ml of semen is needed for pregnancy to occur.
Your report shows 5million/ml. As you can make out, it's very low.
Motility is movements of live sperms in semen. It should be more than 50%.
Your report shows 5%. This is also very low.
Normal shape and size or morphology, of live sperms, should be more than 50%.
Your report shows 3%. Again, this is very low.
In the semen, there are 8-9 wbc/hpf, suggesting infection.
This can be treated with antibiotic course, given by your doctor.
So, the semen report is sub-optimal and will need treatment.
