What Does My Stress Test Results Indicate?

Question: Hi Doctor I am 39 year old male person. I have both hypertensive and diabetete but diabetes is in control and not using any medicines. I am using Olmat-20 of 30 mg for my hypertension in the morning after the breakfast. Recently i have done my medical tests and below are results - Stress test - Negative - 2d Echo - Normal - ECG - Negative - CT Coronary Angiograhpy : Normal, arises from RCA Total calcium score 229.6 Calcific plaques in proximal LAD causing mild to moderate narrowing. Cx is arising from right coronary sinus and its coursing behind the aortic root. Calcific plaque in proximal Cx causing mild narrowing. Please suggest if my stress test is normal , can i assume that there are no blockages ? With Regards XXXXXXX

Brief Answer: please see details. Detailed Answer: Dear Mr XXXXXXX 1. A negative stress test means "It is negative for provocable myocardial ischemia". That means it is negative for presence of disease, hence, normal. 2. The LAD has mild to moderate disease on CT coronary angiogram. If this is producing a narrowing of less than 50%, you need to take precautions only which include good control of diabetes, blood pressure, cessation of smoking, weight control and stress management. If this is causing narrowing more than 50% then you will require some preventive medications too for which you have to discuss with your cardiologist. 3. As per current recommendations, all diabetics are candidates for STATINS (cholesterol lowering drugs). You must discuss it out with your physician. Hope this helps. Feel free to discuss further. Thanks for your answer Dr Sukhvinder ji, Question is how do i know the amount of narrowing i.e whether it is less than 50% or more than 50% ? I also get very mild chest pain which lasts for seconds only. My diabetes is always within permissible limit though at higher side. Do i need any angioplasty to confirm the blockage ? Brief Answer: No need to undergo angiogram. Detailed Answer: Dear Sir Your radiologist, who has performed the CT angiogram will be able to tell that, if it is not mentioned in the report. No you need not to undergo conventional angiogram, as your stress test is negative. Sir, a Diabetic is a diabetic irrespective of the control. Your physician will be able to tell if you fit into definition of being diabetic or not (by looking at your reports). A chest pain which lasts for few seconds is not likely to be anginal. Moreover,your angiogram does not have a severe blockade (>70%) and your stress test is negative, hence, this pain is not anginal. Sincerely Sukhvinder
