What Does My Thyroid Test Report Indicate?

Question:
This is for my wife, aged 37 yrs, height 5ft 3inch, weight 63 kg, who has been on Thyronorm for about 8 years. Last couple of readings and dosage are as below:

24-12-2014: TSH = 0.08, FT4 = 1.15, FT3 = 3.2, she continued thyronorm 100 mcg daily dose

15-04-2015 : TSH 0.45 , continued with Thyronorm 100 mcg

9-Feb-2016: TSH =1.58 , FT4=1.29, FT3 = 2.18

Other complaints - feel tired, though she is very active. goes to gym and is generally active through the day

All tests done from same lab(Thyrocare)

Can we try to reduce the dosage little bit to reduce the dependency on medicine?

Hello XXXXXXX Welcome back to HCM,
I've gone through your query and understand your concern for your wife..
I've reviewed all the lab reports..

Now, as per your description and these reports, the last Thyroid Function test (9th Feb'16) was in normal range only, so I've a few suggestions for her..

Her BMI is acceptable for her age at present, please continue the gym and outdoor activities..

In view of the previous trends and the last report, it'd be wise to continue with the 100mcg dose of Thyronorm only for the time being..

Since it's been 7 months, from the last report, so please get a repeat Thyroid Function test along with Anti-TPO antibodies as well as a proper Ultrasound neck examination to look for the Thyroid gland health at present..depending on these findings, we can take a call whether to continue with the same dose or if it's better to taper it gradually..

Don't worry about dependency here as the mechanism doesn't work that way, in simple words, there's a proven deficiency thus it's getting replaced accordingly..

Also you should note, that persistent tiredness can be due to several other reasons, thus it'll be a good time to get a complete Metabolic profile check-up done for her (Hemogram, peripheral blood smear, FBS, PPBS, HbA1C, LFT, KFT, Electrolytes, lipid profile, Vitamin B12 and D3 levels, 12 lead ECG)..
