What Does My Transvaginal Ultrasound Scan Indicate?

Question: On the Second Day of my periods I got my CA 125 test done as suggested by my GYN and result is 45.27 and also went for a Trans Vaginal ultra Sound which says :

Uterus
Appears anteverted in position, normal in Size (83*43*54 mm), shape and echotexture except 8,11 and 17mm size hypoechoic nodular lesions noted in anterior and fundic intramural locations, Likely fibroids. One another small hypoechoic nodule of about 6mm size also noted in posterior wall intramural location.

Endometrial cavity appears central with thickness 6.8mm and triple line.
Utrine cervix appears mildly bulky and normal in echotexture.

Ovaries
Rt. ovary size : 27*14 mm
Lt. Ovary size : 26*16mm
Both ovaries appears normal in size, shape and echotexture
No cystic or solid mass lesion detected in adnexal location.

Trace free fluid is seen in Pouch of XXXXXXX

Urinary Bladder partially filled, grossly normal.

Want to know are my reports normal and why r my CA 125 results more than range.

Thanks for writing to us with your health concern.
Would you let me know in the first place why CA - 125 levels were being measured in your case ?
Why did your GYN suggest it in the first place ?
It is not done for healthy young women without any symptoms or problems.
It is mainly used to assess the progress of tumours in women with existing ovarian cancer.
BY itself, the test means nothing.
It is of most value in following the course of disease progression or remission.
In your case, the ultrasound scan clearly reveals no ovarian mass, or abnormality.
CA - 125 can be raised in a number of conditions.
Levels above 35 are considered high, in your case, it is borderline high.
IN more than 50 % of women, they are raised for non malignant reasons - fibroids, endometriosis, liver disease, simple stress or fever etc.
In your case, I think it is the small fibroid seedlings ( they are too small to mandate any treatment or concern ) can be leading to this marginally elevated level of CA - 125.
So there is nothing to worry.
I would appreciate it if you could upload the ultrasound scan report and the blood test report ( before different labs have different values of normal and abnormal ) so that I can be more sure of my opinion.

Hi Doctor,

I had earlier twice gotten operated for Fibroids and endometriosis in 2005 and 2008 and had my kid was born in July 2009 from c-section.

My CA 125 before both the surgeries was absolutely normal.

Since XXXXXXX 2014 I have been experiencing Before periods severe cramps in lower abdomen and my migraine specially before and during first 2 days of periods is so severe that i have to take injections.

In XXXXXXX 2014 my Doctor XXXXXXX Mangeshikar advised me to get my FSH,LH,DHEAS and Testosterone done. And as per him my FSH was high and asked me to be on XXXXXXX Tablets for 3 months.

Last month i stopped the medicine but during and post the medication also there was no ease in the symptoms.

Have uploaded the current and previous blood test report.

Sonography done in XXXXXXX 2014 that i have attached was also normal, and i don't know the reason why Dr. Mangeshikar asked me to do CA 125 this time.

Dr. XXXXXXX Mangeshikar is one of the most reputed and knowledgeable people in the country, I am sure you are in the best of hands.
You can ask him why did he ask you to get this test done.
IT is definitely elevated in your case due to fibroids and endometriosis.
In my opinion, you have endometriosis which is responsible for the severe cramps in the lower abdomen, also the menstrual migraine is very typical complaint.
Oral contraceptive pills can treat both of these to some extent.
