Question: hello doctor my ultrasound report are as follows
LMP 21.03.2014
Yolk sac visualized
decidual reaction moderate
internal os closed
cervical length normal\adnexa normal
adv clinical co-relation & follow ups scan

Firstly, I would like to know the frequency of your cycles and if they are regular or not. This is because, presuming that you have 28-30 day cycles, according to your last menstrual period, you should be 8-9 weeks pregnant now.

As per your sonogram report, it appears as though your gestational age is about 5 weeks. Appearance of yolk sac is seen at 5 weeks while cardiac activity can be seen at six weeks. So, to clarify this disparity, you need clinical correlation and repeat sonogram.

The other findings in the sonogram are normal; decidual reaction has been reported to be moderate and this can be reviewed at a later stage and treated if necessary.

Thanks for your reply.
My periods were regular before conceiving. I would like to mention that my doctor has advice for abortion because growth of the baby stopped at 6 weeks & 3 days. Pl help me with your valueable opinion.

It is true that in the current scenario, a missed abortion is diagnosed and abortion done, but, I would ask for a repeat sonogram after 4-5 days along with serial serum beta-hCG titres meanwhile to confirm the arrested pregnancy. Decline in the serum titres instead of the doubling seen every 2-3 days at this stage of pregnancy along with no evidence of progress in the sonogram is confirmatory. Once confirmed, induced medical abortion under surveillance with follow up usually suffices.

Thanks for writing again.

I suggested getting serial serum beta-hCG titre assessment, i.e., you should get the titres measured every 2-3 days for about a week. If there is a fall it is conclusive that it is an abortion; in normal states, the titres double every 2-3 days.

In addition, a repeat sonogram after 4-5 days will help to aid in the diagnosis.

The hCG values at present correlate with about 6 weeks gestation.

After all the tests, if a missed abortion is confirmed, you have to get the pregnancy aborted under medical supervision and after checking the bleeding and clotting profiles properly.

After an abortion, you should wait for at least 2-3 months before trying to conceive again and in the meanwhile improve your diet and general condition.
