Question: I have SGPT (ALT) of 115.80 and advised by my doctor to take an upper abdomen ultrasound. I have type 2 diabetes with FBS level of 135.55 and cholesterol level of 280.74, Triglyceride of 188.22, HDL of 74.41, LDL of 182.69, VLDL of 23.64, Chol/HDL ratio of 3.77. I also do not drink alcohol but am taking maintenance medicines for cholesterol (Indapamide, Gemfribrozil and metoprolol), sugar (Metformin), Vitamin B complex for numbness of hands. What could possibly be wrong with me?

I would explain as follows:

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

Regarding your concern, I would explain that your elevated SGPT levels could be related to a liver disorder (chronic hepatitis, steatosis, biliary stones, etc.).

That is why your attending physician has recommended an abdominal ultrasound to examine the liver and biliary tract structure.
Further liver function tests are needed (gama GT, ALP, LDH, total protein, inflammation tests, bilirubine) to investigate for a liver disorder.

From the other hand gemfibrozil is known to cause an elevation of liver enzymes as an adverse effect.

If all the above tests result normal, I would recommend stopping gemfibrozil for a couple of weeks and repeat liver function tests.

Your blood lipid profile seems to be OK. You have a low cardio-vascular risk based on total cholesterol/HDL ratio.

You can consider the use of atorvastatin or simvastatin instead of gemfibrozil, which are more helpful in decreasing LDL levels.

Hope to have been helpful!

Kind regards,

Dr. Iliri
