What Does My Ultrasound Scan Report Indicate?

Question: Hi,I am a 41 year old mother of two. I have had irregular bleeding for the past four months. a two week period, no period for 45 days and two sessions of bleeding from 4-7 days in one month. Now I'm 33 days with no period. Ultrasound was ordered in beginning of november. Showed complex right ovarian cyst 2 cm. and heterogenous myometrium. Doctor said possible adenomyosis. second ultrasound last week shows two more complex cysts on left ovary and same complex cyst on right side. And still heterogenous myometrium. Is there something else I should be doing? MRI? Endometrial biopsy? I also have very low ferritin levels from recent bloodwork. Not able to run like I could for exercise the last summer. Feel tired. Have had uterine contractions, as well, not cramps but fluttering.

The irregular bleeding that you are having can be due to hormonal changes in the perimenopausal age group. The complex cysts and heterogenous myometrium is suggestive of adenomyosis. If the endometrial thickness in both the scans is normal then there is no need to go for an endometrial biopsy.
You can go for an MRI scan after 4-6 weeks to monitor the size of cysts. In case the size and number of cysts is gradually increasing then CA 125 levels need to be evaluated to rule out ovarian malignancy.
The weakness and tired feeling that you have is likely to be due to low levels of ferritin. Iron supplements will help you in feeling better.
