What Does My Ultrasound Scan Report Indicate?

Question: ultra sound March 21, 2017. Primary biliary cirrhosis stage 4. More compensated than decompensated.
Portal vein up to 8mm in diameter with normal directional flow. Is that ok?
Meld score of 13 but Dr. said Coumadin raises the meld score and if he remembered he would not have ordered the test I think the test was feto-alfa protein l3. Do you agree that Coumadin gives a false reading on the upside? The ultra sound showed NO ASCITIES but my belly has changed shape and [ larger waist].
Please comment.
Summary of ultra sound
1 Coarse heterogenous echo texture liver parenchym explain.
2 Adenomyotosis.

Spleen is normal size and the liver is 14.5 mm[ nurse that is slightly enlarged. Do agree?
CBD measures 4-5 mm but distal evaluation obscured by bowel gas.

-Compensated cirrhosis means that patient is asymptomatic and chances of survival even without liver transplantation are excellent.
-Decompensated cirrhosis is defined by the development of jaundice, ascites, variceal hemorrhage, or hepatic encephalopathy. Survival is poor in patients with decompensated cirrhosis and they should be considered for liver transplantation.

-Diameter of portal vein is normal and flow is toward liver which is a good sign.

-MELD score depends on value of INR. INR is effected by doses of Coumadin and other anticoagulants.As value of INR is effected test results will be non accurate.
So i agree with your primary consultant on this opinion. MELD results can be elevated if INR is grossly deranged.

-Larger waist can be due to a number of factors including obesity.Ascitis has its own classical features signs and symptoms just increase in size of waist does not mean that you have ascitis.Even mild ascitis will be reflected in USG abdomen.

-Normal liver parenchyma(liver tissue) is homogeneous(same texture every where through out the liver as all cells are healthy) but damage to liver(cirrhosis) cause destruction of some cells and part of liver causing Heterogenosity(some part is functioning normal while some is destroyed).

Adenomyomatosis is a benign condition characterized by hyperplastic changes of unknown cause involving the gallbladder wall and causing overgrowth of the mucosa,thickening of the muscular wall.It is a benign condition and no intervention is required.

In a cirrhotic patient mild hepatomegaly has no clinical significance.


Rest all findings are normal.

My gastro Dr. said that the Coumadin alone would raise the meld score and did not mention anything about the effect of the value of the inr. normal inr is 2-3 and mine was 2.2.
I do not understand when you say the meld score could be elevated if inr is grossly deranged. Explain

Life expectancy with more compensated than decompensated.

Maximum amount of Tylenol I could take if necessary.

Does alcohol affect all parts of the liver equally or some areas more than others. Specifically does alcohol affect the bile ducts?

MELD score is calculated by using a formula in mentioned in other answer.If INR is elevated or is effected by medication indirectly MELD score will be affected.

As it is obvious compensated lesion means that there is a problem but is well tolerated by the body while uncompensated means that body can't cope up with the loss incurred hence patient will be symptomatic.
Compensated lesion-Normal life just restrictions are imposed.
Uncompensated- Depending on severity of symptoms upto a decade.
1.2 grams per day however try to take not more then 500mg once a day.

Alcohol effects all parts equally but some area is damaged more while other is damaged less leading to heterogeneous findings.Which part will be effected more can not be predicted.

Alcohol effect the liver as a whole as fat metabolism is hampered,Fat begins to saturate in duct and bladder which can lead to stone formation later.
