What Does My Ultrasound Scan Test Report Indicate?

Question: Hi Doctor
I am 29 years old, unmarried woman
Got ultrasound done in March along with pcod panel test. I have hair on chin and went to the skin specialist for laser tx he advised me to get these tests done first.
Pcod panel test reports are normal
All the hormones are within limit and fine
However, my ultrasound showed the following finding. Please suggest me for the same. I am worried about my fertility too
My height is 5.4 and weight is 52 kg

" both ovaries show multiple small peripherally arranged follicles with excess stroma"
Left ovary measures 4.1 x 2.1 x 2.0 volume 9.5
Right ovary measures 3.7 x 2.3 x 2.2 volume
10.5
My average period lenghth is 38 days

Your sonogram findings suggest the presence of polycystic ovaries, typical of PCOD. It is good that your hormone assays are normal. Since you are getting regular cycles, it is also likely that you are ovulating, in which event, fertility remains good.

You also need to test for insulin resistance and if necessary start taking Metformin. This controls insulin resistance and enhances natural ovulation. This can be continued even into pregnancy also.

Your weight should be within a BMI of 19-25. At present, your BMI is 19.69. Please try to maintain this along with healthy diet and regular exercise.

Please do not worry about future fertility. PCOD is such a common problem now-a-days that several treatment modalities with good success rate are available.

But my pcod panel test included insulin tests as well and that was normal, should I still take metformin

Metformin is a safe and effective drug in PCOD and is often given on an empiric basis in PCOD. It is your consulting doctor's discretion whether to give Metformin with normal levels of insulin.

With proper management and healthy life style, spontaneous ovulation can be attained in PCOD. The sonogram findings may then change, but, the hair growth is irreversible. You may need to use medication or permanent hair removal methods to get rid of it.

You can get your next sonogram done in September. There is no hard rule but usually follow up is suggested after six months. If possible, you can get a trans-vaginal sonogram for a better view of the uterus and the ovaries, though a good pelvic sonogram also suffices, since you are unmarried.
