What Does My Ultrasound Scan Test Report Indicate?

Question: Dear doctor

I did an ultrasound today report showing empty gestational sac measures 22mm and not even yolk sac ..I have irregular periods and I think I ovulated late..can I hope something good or is it a failed pregnancy..??

I had a previous scan 7 days before and showed empty sac measured 19mm

My hcg was 9227 on XXXXXXX 19 and Feb 2 it was 18950...its not doubling but rising

Welcome to HCM,

I have read your question and understood it.

I would like to know when was your last menstrual period.

Technically, we can diagnose a miscarriage only after gestational sac is 25 mm and there no yolk sac or embryo. So it is best to wait to for 7 days

If the scan after 7 days shows no yolk sac or embryo then we will have to consider this as a missed miscarriage.

It is still to early to comment on the viability and there is a small chance that the pregnancy may continue.

Best to repeat scan after 7 days of this scan and then decide. Till then please continue the folic acid .

The HCG is increasing though not as expected, but unless the sac size is more than 25 mm and no embryonic pole seen, till then we cannot comment with certainity.

thanks for your timely reply madam,

my last lmp was on nov 25th but very iregular s,and also i had vit d defieciency and am not taking any tablet now hence i was not ovulating but on nov i have taken one tablet of vit d and next month i conceived,,and after conception i stopped taking it..will that be a reason for this slow development??

i hav hypothyroidim,taking thyroxine 25,my thyroid tsh is 12...folic acid is taking daily...and i have read that sac is growing 1mm per day,but mine was 19mm on feb 2 and feb9th today it is 22mm

in worst case...if i need to do dandc how long i need to wait for the next try??will the same problem reccur??or i need to wait to bring control of my thyroid and vit d level??

Hi,

Vitamin D deficiency is unrelated to ovulation. But it is advisable that you take vitamin tablets till your level is normal again.

Also continue your thyroid tablets, as advised. These do not directly cause miscarraige.

Also if at all there is no growth in next scan, then a diagnosis of miscarriage is certain.

The option you have is medical tablets like mifepristone or misoprostol or a surgical procedure of D and C.

The medicines would be preferable in your case.

You can try immediately for next pregnancy and the continue the medicines for thyroid and vitamin D.

Is this something which will happen again or do we need to wait for 3-4 cycles?

Is there any way that we can try for twins?

Hi,

A missed miscarriage is quite common and can occur in 1 in 6 pregnancies, the cause for it being a defect in the genes of the baby which make it impossible for it to develop.

You should not worry about it as its so common, The chances of it happening again are 15%. That means there is a 85% chance that every thing will be normal the next time.

No point waiting for 2-3 cycles, if at all you miscarry this time, you should try for the next pregnancy when you are mentally and physically ready for the same.

Waiting does not increase or decrease your chances of a successful pregnancy, and the likely hood of miscarriage.

Twins occur in 1 in 80 pregnancy when conceived naturally. They are relatively more common in assisted reproduction.

Twins can be problematic itself with chances of complications being higher in twin pregnancies both in pregnancy, during labor and also after birth. So best to leave things to nature.
