Question: Hi Doctor,
Age 32 yrs, serum creatinine 1.1 mg/dL, ALKALINE PHOSPHATASE - SERUM/PLASMA :73 U/L, URIC ACID - SERUM : 5 mg/dl
My Abdominal ultrasound shows :
both the kidneys are bulky in size and normal in location.
Corticomodullary differentiation is good on left side. no hydronephrosis or calculas is seen on left side.
left kidney :13.0x6.2 cms.
RIght kidney :11.0x4 cms.
Multiple large cysstic structure noted, with loss of CMD and severe cortical thinning, ureter could not be traced.

Thanks for your query ,based on the facts and reports of our scanning that you have posted it appears that you have what is called as Polycystic Disease of the kidney on Rt side which is congenital in origin..

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have high Blood Pressure ?

Based on the scan report you have Polycystic kideny on Rt side .It is always congenital in origin .

You need to consult qualified Urologist /Nephrologist for further investigations
1) DMSA scan to assess the functional status of affected kidney .

Rest of the blood reports that you have posted are well within normal range .

The hypertrophy of the left kidney is compensatory hypertrophy and nothing to be worried about it .

Further treatment will depend upon result of these tests and final diagnosis.

Hi Doctor,
Thanks for answer.
I have hypertension which was detected 2 months back in routine medical check-up. Since then I am on medication for hypertension and diabetes and now both BP and BSL are well within range.
I have few questions -
1. If left kidney swelled compensatorily, what can be its probable life? what precaustions to be taken to extend its life?
2. Diabetologist suggested to visit urologist, he might recommend cystoscopy.
3. Is Polycystic Kidney Disease reversible? what is treatment, precautions for it?
4. What is threat in near future? I do not have any symptomes like pain in abdomen, urination problem etc.

There is no curative treatment for Polycystic Kidney.

Answers to your questions are detailed below
1) The left kidney will have normal life .Only precaution you have to take is that you should not get UTI which predisposes to stone formation .
2) Urologist will assess your urinary system and decide whether the affected kidney needs to be removed or left as it is . I do not think you need Cystoscopic examination .
3)Polycystic kidney is not reversible .It is congenital anomaly and occurs due to diffect in development of kidney in intrauterine life .
4)There is no threat in near future .You have to monitor Blood Pressure and take the medicines to keep it well controlled .

1. Shall I target to reduce left kidney size? or 13 cms is okay for compensatory?
2. Any recommended Urologist from Pune?

Hypertrophy of the left kidney is natural and normal response of the body to compensate the function of the affected kidney.It can not be considered to be abnormal .
