Is it a very bad result ? I do see doctors on a regular basis
I've had many urinary tract infections in the past
Many since surgery in July

the etiology is not clear. There are potential causes for microscopic hematuria but a urinalysis cannot provide the diagnosis. As you've correctly pointed out, infections are a very common cause of hematuria but more abnormal findings are expected (like the presence of white blood cells, nitrates, etc).
Injury of the urinary tract (including a moving renal stone or ureteral stone) may cause bleeding.
Other potential causes include inflammatory diseases of the kidney. Glomerulonephritis is one of them. There are various kinds of this disorder. Clinical and laboratory investigation usually reveal the cause.
Finally one of the most important causes (which should always be excluded) is tumors of the urinary tract. Radiological investigation and direct visualization when feasible (cystoscopy) provide the diagnosis.
So in conclusion, elements of your medical history, clinical examination and more lab tests are required to reach the diagnosis. A radiological assessment of the urinary tract is required to exclude tumors but more investigation would be required for other causes.
