Question: I am 57 and had a few tests and then a vaginal ultrasound that I have a complicated cyst on my ovary. I have been told that my ovary needs to be removed by a general surgeon who has referred me to a gynae, What does this mean?

Thanks for contacting healthcaremagic for an opinion.

If you have been diagnosed with a complicated cyst on a ultrasound, then considering your age group, the scare is to rule out any Ovarian cancer. Complicated cysts are common with cancers and present with thick septations and raised tumour markers.

Firstly, it is requested that you may attach the ultrasound report for perusal to ascertain the size and the location with number of septations.

Next, it would be important to get Tumour markers done in case of complicated cyst namely...CEA, CA-125, AFP and BETA HCG.

If the tumour markers are raised, then a CT-SCAN would be required to look for extent of disease and spread and accordingly plan can be formulated for a surgical staging of the disease.

Please send all the reports and ultrasound as attachments.

A CA125 was done last year which showed it was raised but this test I don't know, General surgeon said results were ok but ovary had to come out. I only have the one ovary left and no uterus. I only have the films but no written report. Cyst was found accidently as I had a gastric sleeve and was having a bit of pain when I shouldn't. andalso some bleeding after intercourse. General surgeon rang obgyn today but office closed for the weekend.

Hello,
If your uterus and one ovary has already been removed , still the tumour markers are necessary to rule out cancer. It is necessary to get a ct scan also done which gives better parameters of ovary and its extent and reconfirmation of the complicated cyst.
If ovarian cancer is detected then not just the ovary but pelvic lymph nodes and bilateral para-aortic lymph nodes would also have to be removed in case of spread.
Also if spread is suspected on evaluation by your gyno , then a PET SCAN would be needed to visualize the lymph node spread before surgery depending on the SUV values.
You may meet up with your gyno after a week , no immediate hurry , and discuss the options in detail,
