What Does My Widal Test Result Indicate?

Question: hi, i have done a widal test, my numbers are s. typhi O - 1:120 and paratyphi A-1:240, S.typhi H and paratyphi B are negative. My physician has prescribed Zifi 200 prior to the test results based on symptoms of high fever for close to 8 days, dry mouth, lack of appetite and intermittent watery diarrhoea. Is further medication necessary? How soon can recovery be?

Thanks for your query.
The Medicine started for Positive Widal is a drug of choice and please continue it for 14 days.
Additional multivitamins and probiotics and Ranitidine help symptomatically.
Paracetamol if fever.
Take full bed rest till you are feeling fit.
Recovery may take 1 to 3 weeks depending upon your response to antibiotic, your resistance power , will power, nutritious diet and bed rest you take.
In few cases Additional antibiotic may be added by your Doctor on your clinical evaluation, response to Zifi and so on.
Some patients are advised admission to the hospital if there is severe weakness, dehydration or if the constitutional symptoms of fever asthenia, fever not getting under control are seen. You Doctor knows exactly what is going on in the area you are residing.

Hello again

The doctor has started me on monocef 2 gm injections once daily and discontinued zifi. I got my preliminary culture report today that says no organism identified. Does this mean the Widal was incorrect? Fever intensity is reduced and generally symptoms seem to be improving

Negative blood culture does not mean you are not suffering from a particular disease, typhoid in your case, or Widal was incorrect.

It is still better that you have been started with Monocef 2 gms once a day.
The most important part is you are improving symptomatically and fever intensity is reduced. This is What decides the management.

Hello,

Thanks for the prompt reply. The fever is around the 100 degree mark and returns every 6-7 hours but promptly reduces upon administering Calpol. Appetite has improved and dry mouth also has disappeared. Would you say, the symptoms are improving? How soon do you think before the fever goes away- have 2 doses of Monocef remaining before the follow up with the doctor.

Also, in order to confirm that this is typhoid do we need to do any further testing? Especially to ensure that this has completely gone away from the system?

Hi.

I hope the watery diarrhea is also under control as are improvement in appetite and disappearance of dry mouth. Yes, these are best prognostic symptoms. Fever of 100 returning every 6 to 7 hours still bothers me a bit and is an indication that you may have to continue with Monocef, or your Doctor may add an additional antibiotic
as per his experience in the area you are staying.

It may take a few more days for the fever to settle.

Please do not go for any further tests to confirm that this is typhoid as you are already under the course and the reports now may be varied and confusing. The clinical judgment by your Doctor has an important part to play.

The Widal test may remain positive for 6 more months in spite of you being cured and this is the mistakes some patients or even Doctors may do. This test is based on the antibodies produced by the body in response to Typhoid and hence can be positive for a long time.

You may please discuss about the elimination of the bacteria from the body with your treating Doctor. In olden days we used to give Amoxicillin for 2 to 3 weeks to eliminate the bacteria from the gall bladder, the organ where the bacteria would be dormant. Now with the recent developments of newer drugs and response to different drugs, it is up to our treating Doctor to decide about this.

I would be really happy to know what your Doctor advises you and be of assistance to you.
